Radiographic techniques for lower limbs Femur The student should learn -the positioning of femur -anatomy of femur -shown structures Femur positions 1. femur AP 2. femur lateral 3. femur medial 0blique 4. femur lateral oblique -femur AP Position 1. the patient is placed in supine position 2. place femur in center of the image receptor(IR) 3.to show knee and femur in true anterior-posterior projection image, rotate the leg about 5 degree in medial direction. (AP position) Femur AP anatomy -Region (femur) -pathology (fracture and bony lesion of femur) -IR size (14*17 inch) SID (100cm) -central ray (project perpendicularly toward femur) -Respiration (suspended) -collimation Include minimum of knee joint cavity and distal portion and proximal portion of femur. Evaluation 1. femur must not be in rotation but show in true anterior posterior projection image 2. femoral condyles should be shown symmetrically about patella Lateral femur -Position 1. the patient is placed either in lateral decubitus position or in supine position. 2. flex knee of filming side 45 degree and make bilateral condyles of femur perpendicularly to image receptor(IR) 3. rotate to the back of opposite leg of filming side to avoid overlapping with other leg 4. place bottom of IR 5cm from the knee -Evaluation 1. patella should be shown in half side image 2. neck of the femur and grater trochanter must overlap completely Collimation (include femur and knee joint) (Region, pathology, IR size, SID, central ray ,and respiration ) like AP position Lateral position of the femur (Lateral anatomy of the femur) (Femur lateral) Femur lateral oblique -Position 1. the patient is placed in supine position. 2. place the femur in center of the image receptor(IR). 3. rotate the knee and the femur about 45 degree to lateral direction. - Evaluation 1. lesser trochanter of the femur in half-side should be shown separated. 2. parts of neck of femur should overlap with grater trochanter. (Region, IR size, SID, central ray, respiration ) like above Pathology cases Femoral fracture (femoral neck fractures)
