Face and Scalp Mercy College of Ohio PA Program Tyler Gossett, PA-C Scalp Facial Muscles Facial Nerve Preview Nerves of Face Cutaneous innervation supplied by trigeminal nerve (CN V) except for posterior scalp and everything posterior/inferior to mandible Trigeminal nerve (CN V) comes from the pons and has both sensory (GSA) and motor (GSE) fibers It has three divisions Ophthalmic nerve (CN V1)- all sensory Maxillary nerve (CN V2)- all sensory Mandibular nerve (CN V3)- sensory and motor (muscles of mastication and tensor twins) Trigeminal nerve serves as a major highway system that other fiber types can hop on to get to their destination Nerves of Face Trigeminal distribution Nerves of Face Motor innervation of the face is supplied by facial neve (CN VII) (muscles of facial expression) or by previously mentioned CN V 3 (muscles of mastication, “tensor twins”, mylohyoid m., anterior belly of digastric m.) CN VII arises from pons and has GSE fiber and parasympathetic (GVE) fibers and special sense fibers that originate from the nerve Do not worry about the later two and just focus on the GSE for now Forms parotid plexus in the parotid gland and gives off 5 branches Temporal branch of CN VII Zygomatic branch of CN VII Buccal branch of CN VII Marginal branch of CN VII Cervical branch of CN VII CN V3 Tensor Twins: Tensor veli palatini m. (opens and closes the Eustachian tube Tensor tympani m. (tenses the tympanic membrane to dampen sound) Muscles of Mastication: Masseter m. Temporalis m. Medial and lateral pterygoid mm. Arteries of Face Most branching off external carotid artery Lymphatics of Face and Scalp CC Bell’s Palsy Paralysis of CN VII Inflammation of CN VII by herpes simplex, other viral causes, pregnancy (fluid retention), idiopathic CC Trigeminal neuralgia Compression of CN V (aberrant artery, tumor, cyst, MS, brain lesions, etc) Pain with chewing, brushing teeth, etc Pain is very extreme CC Pulses of Face and Scalp Superficial temporal artery Facial artery (can be hard to find)
