Dysmenorrhea Chapter9 DONOTgiveNSAIDsif havethese they a Dysmenorrhea= painful menstruation Primary Dysmenorrhea: -as soon as 6-12 months after 1st period, but usually several years after -regular cycles w/normal blood loss -onset with beginning of menses & lasts 2-3 days -RESPONDS TO NSAIDS -other symptoms= headache, fatigue, nausea, change in appetite, backache, dizziness, etc. Treatment for Primary OTC-pharm: -Aspirin, Acetaminophen, & NSAIDS (best choice) *NSAIDS= Ibuprofen (200-400 mg every 4-6 hours) Naproxen (220-440 mg initially, then 220 mg every 8-12 hours) Selenium *Aspirin= 650-1,000 mg every 4-6 hours *Acetaminophen= 650-1,000 mg every 4-6 hours Secondary Dysmenorrhea: -at least 2 years after 1st period, more typically mid to late 20’s or older -irregular usually w/intermenstrual bleeding -pattern & duration varies -can occur before, during, or after menses -NSAIDS NOT USUALLY ENOUGH -other symptoms= vary but can include dyspareuria or pelvic tenderness MS Chapter on Premenstrual Syndrome= at least 1 mood or physical symptom during 5 days prior to menses Premenstrual dysphoric disorder= 5 or more symptoms, with one being mood symptoms gone week after period Premenstrual exacerbation= worsening of psychotic symptoms, other conditions like IBS, asthma, etc. will worsen premenstrually **no symptom free interval Premenstrual symptoms: -Negative= fatigue, lack of energy, irritability, anger, labile mood/alternating sadness & anger, depression, anxiety, feeling stressed, crying spells, oversensitivity, di culty concentrating, abdominal bloating, edema of extremities, breast tenderness, appetite changes, headache, hypersomnia, insomnia, joint/muscle pain -Positive= increased energy, more e cient at work, increased libido, more a ectionate, increased sense of control, more self-assured OTC-pharm: ~Pyridoxine ~Calcium (600 mg BID) & Vitamin D ~Magnesium ~NSAIDS= for pain! ~Diuretics (FDA approved)= for bloating!! 3 main ingredients: Pamabrom, Ca eine, Ammonium Chloride (all help w/bloating) EX of drugs= Aqua-Ban (Pamabrom), Diurex (Ca eine), Pamiprim (Pamabrom &Acetaminophen), Midol (Ca eine & Acetaminophen) **Combination is the popular therapy!!
