CASE MANAGEMENT (November 2023) CONFIDENTIALITY 1 WHY IS CONFIDENTIALITY IMPORTANT? Confidentiality is the foundation of the work we do. Must protect the dignity of the JM at all costs. JM must be able to trust us when sharing personal information. Confidentiality is the right of the individual we work with. Confidentiality helps us to avoid stigma. Confidentiality maintains institutional credibility. 2 CONFIDENTIALITY: KEY RULES 1. Do not share information with anyone except your Case Lead. ○ Protect JM information shared in trust. ○ Any personal notes you may have taken must be destroyed after being updated in the system. 2. Confidentiality applies to everyone. ○ Including volunteers, teachers, staff members, leadership. 3. Confidentiality is timeless. ○ It does not end when you complete your task / role with a case. 4. Consented sharing. ○ Be transparent about what, when, how, and with whom information will be shared. 3 WHEN CAN YOU BREAK CONFIDENTIALITY? If a Jamati Member provides informed consent. If a Jamati Member indicates thoughts or intents to harm him/herself. If a Jamati Member indicates thoughts or intents to harm someone else. If a child, elderly person or other vulnerable person is being abused, neglected or in danger. Remember: When breaking confidentiality, follow escalation strategies; sensitive information should stay within chain of escalation. 4 ESCALATION STRATEGY 1. We have a simple escalation strategy. ○ Always escalate to your case lead. 2. What if the lead doesn’t respond or isn’t available? ○ Escalate to their lead and/or the On-Call Case Manager. 5 Is this Breach of Confidentiality? Leaving JM call notes on your kitchen fridge to add to CMS later Your spouse is part of EPB. JM needs help with employment you reach out to your spouse for consultation Writing case details in an email including JM name and/or their family member names Sharing case details with your team members who are not involved with the case MK asked you about a family because they know you are a family mentor, and it happens to be your case, so you provide an update Seeking second opinions from family, friends, team members who are not involved in the case Referring to a case in an email via case number Chairperson from Youth & Sports Board referred a family to Access. A month later the Chairperson approaches you for information about the family 6 CASE MANAGEMENT QUESTIONS? 7
