WEEKLY CREW TIME SHEET - CORRECTION NEEDED F-398 INITIATED BY Supervisors of Payroll Divisions NUMBER OF COPIES Two PERIOD RETAINED By Accounting Services Section until usefulness is past. FORWARD Both copies to Accounting Services Section by the requesting member’s supervisor. REFERENCE Manual of Operation, Vol. 1 0/7-03.00 and Vol. 5, 9/7-00.00, Form #5054 (Positive Timekeeping), Form #5054 (Firefighter Exception) and Form #5054 (Firefighter Overtime). ROUTING To Station/Unit Commander for approval. The Accounting Services Section shall return the approved duplicate copy' which is to be filed in the effected member's personal folder. APPLICATION: A. This form is to be used to make changes on Weekly Crew Time Sheets Form #5054 (Positive Timekeeping), Form #5054 (Firefighter Exception) and Form #5054 (Firefighter Overtime). B. Entries: 1. Check appropriate box in heading to indicate which type of Weekly Crew Time Sheet is being corrected. 2. ENTER PAYROLL DIV., PAY PERIOD, WEEK ENDING DATE, EMPLOYEE IDENTIFICATION NUMBER and EMPLOYEE NAME exactly as it appears on the incorrect Form #5054. 3. Show the INCORRECT ENTRY exactly as it was submitted on Form #5054. 4. Show the CORRECTED ENTRY exactly as it should be. 5. Other - Give a brief explanation of the correction being made i.e.: a. Engineer Boncolce was on TS/24 time instead of VC/24 time on that date. b. Captain Crammer's FI/12 was not approved; therefore he took TS/12 time on that date. c. Firefighter Jones received a VC/24 on a date that he was actually on duty; therefore no entry should be shown. d. Firefighter Peters received a TS/24 that should have been entered for Firefighter Wintor. A F-398 will also be forwarded to correct Firefighter Wintor's entries. e. Captain Cook was off 24 hours AOD time instead of TS/24 on that date; therefore no entry should be shown. NOTE Do not forward any F-398's correcting a TS/24 without indicating why the member was off duty. 6. Signature of Affected Member - Assignment. To ensure that the member whose timekeeping is being adjusted is cognizant of, and approving of, the timekeeping correction being made, the affected member must sign on the Signature/Assignment line. Supervisor may not sign for the affected member. 7. Signature of Station/Unit Commander – Assignment. Supervisors responsible for timekeeping shall approve and sign the form. Members may not approve their own timekeeping corrections.
