HOFFMAN ESTATES FIRE DEPARTMENT STANDARD OPERATING GUIDELINES SOG Category & Identification Number: EFFECTIVE DATE: OPERATIONAL GUIDELINES – 006 January 1, 2022 NIMS COMPLIANT SOG Title: Forcible Entry Report and Notification Revision: 3 APPROVED BY: NUMBER OF PAGES: Re-evaluation Date: Alan Wax January 1, 2024 Fire Chief 2 PURPOSE DEFINITIONS Whenever the Fire Department forces entry into a building, a residence or a privately owned vehicle and the resident/owner is not on the premises, fire personnel are responsible to leave a forcible entry notification and complete the forcible entry worksheet in NFIRS. POLICY The Company Officer or Paramedic (if only EMS personnel are on the scene) involved in the forcible entry operation is responsible for utilizing the notification form and completing this report, along with requesting that the Police Department respond to the scene where forcible entry will take place. The company officer or senior paramedic is to complete the Forcible Entry Worksheet within the NFIRS report. A yellow premises notification form is located in the apparatus report box. The Company Officer or Senior Paramedic is to complete the notification form and attach it to the access door of the building or placed inside the vehicle that was forced into. A minimum of 10 notification forms are to be kept in the report box of each engine, squad, and truck. During daily inspection of the apparatus, the engineer is to determine the number of notification forms in the report box.
