GENERAL.01 ALTERATIONS AND INSTALLATIONS For installation of telephones, buzzer systems, public address systems, and bell systems, forward an F-225, through channels, to the Assistant Bureau Commander, Training and Support Bureau. F-225 Forward As indicated To Assistant Bureau Commander, Training and Support Bureau, through channels Number of Copies Two Routing Upon approval/disapproval, original retained at Bureau, duplicate returned to point of origination for filing. Information Required Specific description and justification for the requested equipment. Attach a diagram indicating exact location(s) within the facility, where the equipment is to be installed..12 REPAIRS Members shall not attempt repair on tables, chairs, benches, or other equipment. Such equipment must be sent to Supply and Maintenance Division for repairs. Refer to Vol. 4, 8/3-16.56. Repair requests for typewriters, facsimile machines, and calculators shall be to submitted through Network Staffing Services (NSS) to Building Administration Unit. Repair requests for computers and computer related accessories shall be telephonically submitted directly to Management Information Systems (M.I.S.)..24 VENDING MACHINES No installation which provides an income shall be permitted in quarters unless the income is used for the benefit of all members working in the station..36 STATION 'GRAPEVINE' TELEPHONE CHANGE OF NUMBER Changes in grapevine telephone numbers shall be reported in writing by Station Commanders to the Community Service Unit without delay..48 LOITERING Members shall not congregate or loiter in front of or in the entrance to Company quarters to an extent that will cause criticism to be directed at the Department. 4/4 QUARTERS 2.49 SMOKE DETECTORS Smoke Detectors are to be maintained as follows: A. Smoke detectors are to be tested monthly. B. Clean the detector every time the battery is replaced. Clean dust from sensing chamber and from all the openings in the cover. C. Each January, Battalion Commanders will order batteries for smoke detectors mounted in fire stations in their command. D. Whenever a smoke detector unit becomes disabled, a replacement will be procured by tagging with an F-175 and forwarding to S&M..50 STATION RESIDENCY The privilege of establishing residency in a Fire Station shall require approval of Bureau Commander, Emergency Services Bureau (F-225). F-225 Forward As indicated To Bureau Commander, through channels. Number of Copies Two Routing Upon approval or disapproval, original retained at Bureau office, duplicate return to point of origin. Information Required 1. Name and rank 2. Station when residency will be maintained. 3. Effective date. Members shall submit an additional F-225 when they discontinue station residency or change stations. The member's superior officer shall attach an F-225 to the original report which shall include the following information: A. Confirmation that the officer has consulted with involved officers of opposite platoons. B. Opinion as to the effect such residency will have on morale and Company operations on each platoon. C. Any additional considerations which provide the basis for approval or disapproval. For the purpose of this section, station residency is defined as residency of a permanent or semi-permanent nature to the extent that records of the Department reflect such location as the official address of the member. 4/4 QUARTERS 3.55 USE OF FIRE DEPARTMENT PERSONNEL OR FACILITIES BY OTHER AGENCIES Any requests for use of Department facilities or personnel by members of other agencies such as the Los Angeles Police Department, Federal Bureau of Investigation, Department of Transportation, Building Services, City Attorney, etc., shall be directed through channels to the Administrative Office for approval. Normally, these requests should be in writing but time critical requests may be made telephonically, through channels, to the Administrative Office. Members shall not commit the Department for use of facilities, personnel ride-alongs, etc., without appropriate approval..60 ENERGY CONSERVATION Energy conservation shall be the responsibility of all members. Battalion Commanders shall ensure that the following minimum measures are met: A. Heating and Air Conditioning 1. Water heating appliances shall be set at a maximum of 140 to 150 degrees Fahrenheit. 2. When the ambient temperature is between 66 and 80 degrees Fahrenheit, open windows and discontinue the use of power ventilators where possible. 3. Adjust heating and air conditioning thermostats to start heating when temperature drops below 68 degrees F. and cooling when temperature is above 78 degrees Fahrenheit. Window units should not be turned on until temperature is above 78 degrees Fahrenheit. B. Lighting 1. Gong lights shall be turned off when leaving quarters on responses..72 WEARING APPAREL When not in use, members shall keep all wearing apparel and other personal belongings in a locker, closet, or other proper place provided for this purpose. The locker or closet shall be kept in a neat and orderly condition at all times and the doors kept closed and locked. City or personal property shall not be placed or hung on the apparatus in such a manner that it will be detrimental to the finish or use..84 PRIVATE CARS Privately owned automobiles will not be permitted in station yards without permission of the Station Commander. In granting this permission, consideration must be given to the cleanliness of yards in regard to oil and grease deposits. Premises shall be maintained in an orderly condition. Mechanical work of any kind on private automobiles is prohibited on Department property. Storage of any private vehicle in or on Department facilities is allowed only with written approval of the Bureau office. 4/4 QUARTERS 4.86 FACILITY SAFETY INSPECTION Annual Facility Safety Inspections shall be conducted by Division Commanders at all work locations within their command, utilizing the Safety Inspection Checklist. These inspections will be recorded in the station or unit journal and the checklist filed and retained at the inspected facility for five years. Refer to Book 75, Accident Prevention and Safety Manual, Facility Inspections..90 BATTALION INSPECTION Inspection shall take place every other month by a Battalion Chief or higher commander. Inspections should normally be limited to one weekday in order to minimize the impact on other activities and weekend training. Inspections shall take place bi-monthly, within the first two weeks during even months. Items to be inspected may include: All assigned apparatus Quarters Personal Safety Equipment Equipment and areas designated by the Battalion Commander NOTE Refer to Training Bulletin #127 for Inspection Cycle Information. The Battalion Commander may in addition to the above, designate a particular area of “emphasis”. The inspection of portable hand-held radios shall be included with members’ personal protective equipment. Radio equipment that has been altered, damaged, or in general poor condition may lead to failure in the member’s ability to communicate. All apparatus with an F-377 or an F-377RA due, shall have the forms completed and available for Battalion Commander review. It is the Station Commander’s responsibility to ensure that apparatus maintenance, F-377 reports and post inspection work is properly completed in a timely manner. When applicable, Battalion Commanders should utilize representatives of the Engineer and Apparatus Operators Network Program to assist with correcting deficiencies. Reports and records will be inspected as needed during routine Battalion Commander visitations. Inspections shall be rotated according to the following schedule: "A" Platoon "B" Platoon "C" Platoon February April June August October December.96 DEPARTMENT-WIDE INSPECTIONS Following is a list of items which shall be checked by Company Commanders prior to Department-wide inspections: A. Check all company records, personal record books, and log books for completeness. B. Check all company files - dispose of those items which no longer need to be retained. C. Conduct a pre-inspection of personal equipment, I.D. cards, driver's licenses, uniforms, and lockers. D. Check bulletin boards - remove items which no longer need to be posted. E. Check library for completeness. Refer to Vol. 1, 0/3-17.00. F. Apparatus G. Quarters 4/4 QUARTERS 5.99 FACILITY SAFETY INSPECTION /INVESTIGATIONS BY OUTSIDE AGENCY If a Cal/OSHA inspector arrives at your workplace, do the following: A. Call the Department Safety Officer immediately at (213) 485-6011. Also, if in Division I call Battalion 2 (213) 485-6272. If in Division II call Battalion 3 (213) 485-6273. If in Division III call Battalion 15 (818) 756-8615. B. Do not give the Cal/OSHA inspector access to the premises until the Department Safety Officer or a City Safety Engineer arrives. Refer to Book 75 page VI-2 and City of Los Angeles Safety Bulletin No. 2. -17. STATION MAINTENANCE.01 COOPERATION Station and Company Commanders shall confer with their counterparts on opposite platoons with regard to repairs, supplies, and upkeep of quarters. They shall work harmoniously together in order to ensure that responsibility and work load are shared equally, among platoons, insofar as practicable..14 HOUSEWORK Station Commanders shall establish schedules for the cleaning and maintenance of quarters and apparatus. The entire premises, including roofs, gutters, sidewalks, lawns, shrubbery, etc., shall be maintained in a neat and clean condition at all times. The assignment of these duties shall be equalized as nearly as possible among the members..42 FLOOR MAINTENANCE Asphalt Tile Floors Will require the application of vinyl base wax only. Quarry Tile Floors Will have a sealer applied following installation and should only require damp mopping. Carpeted Floors Will require vacuuming as needed. A vacuum cleaner is provided by the Department to those stations having carpeted areas. When carpeting is in need of cleaning, Station Commanders shall contact the Building Administration Section, who will make arrangements with a private contractor..56 FLOOR POLISHERS When not in use, floor polisher brushes shall be removed and stored in such a manner as to prevent any damage to the bristles of the brush. Polishing brushes will not be used for scrubbing floors under any conditions. Alteration of the automatic switches on floor polishers to prevent automatic shut-off is strictly prohibited..70 SPONGES AND CHAMOIS Sponges and chamois which are used for cleaning purposes in quarters shall be kept available for use of all members. When not in use, they shall be properly stored to allow for drying. 4/4 QUARTERS 6.84 WIPING TOWELS Whenever wiping towels have been used to clean batteries or have been in contact with battery acid, such towels will be thoroughly rinsed with fresh water and dried before using again or stacking with other towels..99 RENOVATION OF FIRE STATION LAWNS Officers desiring to obtain this service shall contact the Department's Landscape Coordinator direct. The Fire Facilities Coordinator may be contacted for the Landscape Coordinator's assignment. -34. SECURITY OF QUARTERS.01 GENERAL At all times one member shall be designated as the station watch. After ascertaining that the station is locked and secured, they shall not be restricted as to location within the station, or as to activity, so long as they are promptly available, in non-athletic uniform, (except when the entire company is engaged in physical fitness activities) to answer the doorbell..05 SECURITY RESPONSIBILITY Station Commanders have the overall responsibility for station security. Each member shares station security responsibility..07 SPECIFIC MEASURES A. Quarters shall be secured upon response and at any time they will be unoccupied. B. Members shall keep their personal lockers locked. C. Members shall be alert upon returning to quarters for evidence of unauthorized entry. D. Fuel storage and use 1. Fuel pumps shall be kept locked at all times when not in actual use. 2. Remote electrical switches to fuel pumps shall be turned off when not in use. 3. Underground tank fill caps shall be kept locked..11 STATION WATCH ASSIGNMENT Station watch assigned to non-officer members shall be equitably divided. A station watch roster shall be posted. Provisions shall be made to reassign the duty when the assigned member is required to be absent. When more than one door must be watched, or other station security or public contact duties require, the station watch shall call for the necessary assistance..20 STATION WATCH DUTIES The station watch shall have station security and public contact as their primary responsibility. Other duties include: A. Keeping alarm appliances ready for use (this includes daily door bell test). B. Receiving teletype messages. C. Actuating answer-back on receipt of taps or special signals. D. Calling a Company Commander when conditions arise that they cannot handle, when supplies are delivered, and when workers arrive on City business. 4/4 QUARTERS 7.22 THEFTS/BREAK-INS OR SUSPECTED THEFTS/BREAK-INS - QUARTERS The intent of Station/Facility security is to protect City property as well as the property of assigned members. Parking lots, and the vehicles contained, are part of a Fire Department facility. Upon discovery, or report, of a theft/break-in or an attempted theft/break-in at any Fire Department facility, there shall be an immediate investigation by a Chief Officer, preferably the Administrative Battalion Commander. The on-duty station commander, or other responsible person, shall: A. Ascertain whether or not there has been a theft/break-in, or an attempted theft/break-in. B. Immediately notify Metro, requesting an investigating officer and Los Angeles Police Department response, for filing appropriate reports. Metro will dispatch a Battalion Commander and assign an incident number to the investigation. C. During regular business hours, the appropriate Bureau Commander shall be notified immediately, through channels, by telephone. For incidents occurring at times other than normal business hours, this notification shall be made at the start of the next business day. D. Preserve the scene/evidence and gather all pertinent information which may assist the investigation. E. Forward an F-225, THROUGH CHANNELS, to Bureau Commander. F-225 Forward As indicated To Bureau Commander, THROUGH CHANNELS Number of Copies Three Reference 9/1-01.20 Routing Original Copy - Bureau Commander Duplicate Copy - Operations Triplicate Copy - Department Security Officer Information required 1. Narrative containing details of incident. 2. Security status of missing article(s) and facility at time of theft/break-in. 3. List missing article(s) and/or damage(s); estimate dollar loss: i.e. repair and/or replacement cost for each item. 4. List of all members on duty at time of incident and their permanent assignments. 5. All information relative to the incident that could assist the investigating officers. 6. L.A.P.D. investigating officer's name, I.D. number, unit designation and the P.D. report number (DR #). If available, a copy of the Police Report shall be attached to the F-225. 7. Name and assignment of investigating Battalion Commander. Incident number assigned to investigation. F. Forward subsequent reports as requested by the Operations Executive Officer. 4/4 QUARTERS 8 The investigating Battalion Commander shall forward an F-225 to Bureau Commander, THROUGH CHANNELS, detailing the results of the investigation. The investigating Battalion Commander shall also make recommendations. Note F-225's shall be forwarded by the end of the next working shift/day. Operations will review each incident for the purpose of determining whether a hearing should be held..40 AUTOMATIC DOOR OPERATION A. The door timer and operation shall be checked each morning. B. All members on duty shall be thoroughly familiar with the automatic door operations. -51. PUBLIC CONTACT.01 GENERAL Station Commanders shall cause those persons coming to a fire station at any hour on business or as visitors to be promptly greeted..11 ENTRANCE PROHIBITED Officers may prohibit the entrance of, or cause any person to leave quarters. Entrance to quarters for the purpose of soliciting is forbidden in accordance with the Rules and Regulations. Loitering in quarters is prohibited. Printed notices to this effect shall be posted so as to be visible to persons entering..21 FIRE STATION VISITING HOURS No visitors are allowed after 2100 hours except for special events (Department handball tournaments, civic dinners, etc.), when the hours may be extended to 2300 hours..31 VISITORS Visitors, after identifying themselves, will be escorted through quarters for necessary purposes. A desire to view apparatus and quarters is a necessary purpose. Children must be accompanied by an adult. When their presence will not interfere with Fire Station activities, the following should not be restricted from quarters during normal visiting hours: A. Member's families visiting members working in the station. B. Civilians rendering necessary service to members on duty. C. Public employees entering on business. D. Boy Scouts visiting in accordance with 4/1-84.00 of this Manual. NOTE If, in the opinion of the Station Commander, importance of a visitor warrants official recognition, the Community Liaison Officer will be notified by telephone. This information will be forwarded to the Administrative Office for any action deemed necessary. 4/4 QUARTERS 9.33 VISITOR REPORTS A. Accidents/Illness or Injury: When persons who are not City of Los Angeles employees suffer accident, illness or injury while on City property, complete and forward a Form General 87 to the Administrative Office, through channels, for forwarding to the Personnel Services Section. Refer to Vol. 5, 9/7-00.00, Form Gen. 87. B. Groups - Important individuals: Complete F-971 and forward. Refer to Vol. 5, 9/7-00.00, F-971. C. Waiver or Indemnity Agreement - Visitors subject to possible injury shall sign a Department F-1109A or F-1109B. The circumstances under which injury may occur and approvals required are described on the F-1109A and F-1109B. Refer to Vol. 5, 9/7-00.00, F-1109A and F-1109B..41 MEMBER'S IDENTIFICATION Members off duty who enter fire stations must identify themselves to the Commanding Officer on duty unless their identity is known. -68. BEDS.01 BEDS NOT OCCUPIED During the time a bed is not occupied (normally between 0630 and 2000) it shall be dressed with the Department issued bedspread only. The bedspread shall be pulled even with the headboard..36 BED OCCUPIED During sleeping hours when a bed is to be occupied, it shall be dressed with only the member's linen. All linen and bed clothing shall be kept in a neat and sanitary condition. When a bed is dressed for sleeping, the bedspread shall be removed and stored in a clean place. Sleeping on top of bedspreads is prohibited..45 TURNING OF MATTRESSES This procedure is established to get maximum life from mattresses issued to Fire Stations. All Department mattresses are to be marked with numbers 1 through 4 using a black felt tip marker (some mattresses are already marked). Each unmarked mattress is to have a 3" high number placed as near the top edge as possible, equidistant from the sides, as shown in the accompanying drawings. Mattresses shall be turned on the first day of each calendar quarter. On January first the numeral "1" is to be at the head of the bed. On April first the numeral "2" and so on. Mattresses shall be turned by two members to avoid bending and tearing the mattress. 4/4 QUARTERS 10.50 BED NUMBERING Beds shall be numbered by the use of decals. Numbers are to be centered on the foot of the bed..54 SLEEPING Members on duty should retire between 2000 hours and 2300 hours and arise not later than 0630 hours. Sleeping on duty is normally prohibited between the times of 0630 hours and 2000 hours..63 OFF - DUTY MEMBERS Members who are off-duty and desire to occupy beds in quarters shall obtain the permission of the Station Commander on-duty in every case. Refer to Vol. 2, 4/4-01.50 for station residency..72 SLEEPING ATTIRE Members on duty shall not retire in any portion of the uniform, but must be in sleeping attire with turnouts adjacent to bed..81 SLEEPING DISTURBANCES Members occupying beds in quarters shall not be unnecessarily disturbed..90 ELECTRIC BLANKETS The use of electric blankets is allowed in stations where adequate electrical facilities are available. If extension cords are used, they must be legal and kept out of the flow of foot traffic. All extension cords must be picked up each morning. -85. BULLETIN BOARDS.01 CLASSIFICATION Bulletin boards are of two classifications - "Official Business" and "General Information." One or more bulletin boards shall be designated as the official business bulletin board(s)..33 OFFICIAL BUSINESS BULLETIN BOARDS All "Official Business" bulletin boards shall be so labeled and used only for posting official Department material, material issued through channels that applies to Department operation, or material that is of direct official interest. Station Commanders shall post on the bulletin board, for a period of 30 days, communications that may require the further attention of personnel. After this period, these communications shall be filed in a convenient place for future reference and shall be kept until their usefulness is past..48 CAL/OSHA POSTING The "Official Business" bulletin board will be used to permanently post the Cal/OSHA information poster and to post the Cal/OSHA No. 300 A (Summary) during the month of February..66 GENERAL INFORMATION BULLETIN BOARDS All "General Information" bulletin boards will be used for posting material of general interest to members whether distributed through channels or not. Responsible officers shall screen material to be posted on these bulletin boards. They shall not be used for posting of highly controversial or offensive material. 4/4 QUARTERS 11.70 COMMUNICATIONS FROM RECOGNIZED EMPLOYEE ORGANIZATIONS The Department will provide a bulletin board or space for the exclusive use of posting communications from recognized employee organizations. All official communications from recognized employee organizations shall be posted in the space provided. Recognized employee organizations shall cause a removal date to be placed on all material posted. Said removal date shall be not more than 60 days from the date of posting, unless otherwise agreed to by the designated Department Management Representative. -91. LOUNGE CHAIRS.01 GENERAL Lounge type chairs may be purchased by members provided that: A. Prior to purchasing any lounge type chair, an F−225 be forwarded through channels and approved by the appropriate Bureau Commander. B. The F−225 request shall be signed by all Station/Unit Commanders, Battalion/Section Commanders, and Division Commanders within the appropriate chain of command. C. The F−225 request for approval shall list the number of chairs to be purchased and indicate in what room or area they will be used. A picture of the chair, the chair's overall measurements, and a sample of the chair covering material will be attached. D. All lounge chairs in any single work location will be the same style and made of the same covering material. E. Lounge chairs will not be mixed with aircraft type chairs in the same room or area. F. Department furnished chairs may be used in conjunction with lounge chairs. G. Lounge chairs purchased by members shall be maintained to Department standards. H. Aircraft type chairs shall be maintained to Department standards. -95. SMOKING REGULATIONS.01 GENERAL The smoking policy is as follows: Smoking is prohibited inside any Fire Department facility. -99. RESTROOM, SHOWER AND LOCKER FACILITIES.01 GENERAL Fire Department work locations are to provide a separate gender facility. The facilities shall be labeled accordingly and shall include toilets, sinks, shower, lockers, and changing rooms..10 TOILETS Toilets are to have individual compartments and each shall have doors and latches. “No person shall be allowed to use any toilet facilities assigned to persons not of that gender.” This policy does not prohibit such facilities from being cleaned or maintained by someone not of that gender. 4/4 QUARTERS 12.15 SHOWER ROOMS Fire Department work locations that house showers and changing areas with lockers shall provide separate facilities for each gender. Male employees shall use only those areas labeled "Men." Female employees shall use only those areas labeled "Women." Showers are to have an individual stall with a door and latch. If there is no door or latch, there will be an attached private dressing area. This will allow more than one person to use the facility at a time, as well as affording some privacy if the restroom door is opened..20 LOCKERS AND CHANGING ROOMS Male employees shall use areas labeled “Men” only. Female employees shall use those areas labeled “Women” only. Where possible, at least one locker should remain empty and easily accessible as a SOD locker in each separate gender facility. Storage of supplies or other unnecessary items shall not be permitted in SOD lockers..25 OFFICER RESTROOM FACILITIES Officers of different gender that currently share a restroom shall utilize the privacy lock and if available use the male/female sliding sign to meet the requirement for the "Minimum Privacy Standard.".30 CLEANING AND MAINTENANCE The cleaning and maintenance of restroom facilities is the responsibility of the Station and Company Commanders as stated in Vol. 2, Section 4/4-17.14 and shall be maintained in a neat and clean condition at all times. The assignment of these duties shall be equalized as nearly as possible among the members. As with the cleaning of Captains quarters, the same respect should be displayed in the cleaning and use of separate gender facilities. NOTE Nothing in this policy prohibits men from cleaning the female facilities or women from cleaning the male facilities. However, use of the facilities by someone not of the designated gender is prohibited.
