A goal without a plan z is just a wish. Antoine de Saint-Exupéry Creating z S.M.A.R.T. Goals with cardiac rehabilitation in mind z S.M.A.R.T. goals  Many of you are familiar with creating S.M.A.R.T. goals.  SMART goals can be created for all aspects of your life.  There is a very specific format that works best and will be acceptable for use in this course for our CR patients. Please follow the instructions in this presentation. z Before creating a SMART goal  Assist your patient to recognize their risk(s).  Once CR patients have completed their initial assessments upon entering the Cardiac Rehabilitation program, their risk(s) for subsequent cardiac events will be identified. The ultimate goal of CR is prevention whether it is primary or secondary prevention programs.  It is not the CR team member who creates a SMART goal for a patient; it is the patient who determines their own goals. z S.M.A.R.T. goals  Specific  Measurable  Attainable  Relevant  Time frame z Specific  Start by stating the goal as specifically as possible.  An example of a specific goal might be “lose weight”, as opposed to non-specific goal of “I want to have more energy so I can improve my home and work performance.” z Measurable  Measurable criteria will allow progress to be measured. It will be obvious when a goal has been met.  An example of a measurable goal might be “lose 5 pounds”, as opposed to non-measurable goal of “lose some weight.” z Attainable  Goals should be ambitious but not impossible. Create smaller goals that work towards your long-term desires and wishes.  An example of an attainable goal might be “lose 2 pounds per week”, as opposed to non-attainable goal of “lose 10 pounds per week.” z Relevant  Goals should be meaningful to you, set by you and not set by anyone else (CR team members can assist but not dictate).  Prioritize goals. Not everything can be done at once. Examine your life and decide which goals are:  The most important to you.  Reasonable for you to take on at this point in your life.  An example of a relevant goal might be “lose 5 pounds,” as opposed to non- relevant goal of “learn to speak Spanish.” z Time frame  Choose a time frame that makes sense, not too soon and not too far into the future. Better to celebrate smaller accomplishments often, it keeps up the motivation.  An example of a realistic time frame might be “in the next 30 days”, as opposed to an unreasonable time frame of “in 12 months”. z Pathways to your SMART goal For our purposes, 3 different pathways are required. Some examples might be: 1. Enroll in [specify topic] education session (or watch your classmate's presentation on [specify topic] ). State why/how this will help you reach your goal. e.g. Diabetes, tobacco cessation, resistance exercises, etc. 2. Get a referral for [specify type] counseling. State why/how this will help you reach your goal. e.g. Alcohol, depression, diet, sleep clinic, etc. 3. Join [specify type] support group. State why/how this will help you reach your goal. e.g. Bereavement, MI survivors, Post CABG, etc. 4. Alter [specify which behaviour]. State why/how this will help you reach your goal. e.g. increase my daily walking to 30 min, reduce the amount of saturated fats in my diet, quit smoking, etc. z Choose the properly stated SMART goal 1. Lose 5 pounds 2. Lose 5 pounds by Christmas 2024 3. I want to lose 5 pounds by Christmas 2024 by eating less, exercising more and getting a better-quality sleep. 4. I want to lose at least 5 pounds before I take a vacation. z Choose the properly stated SMART goal The correct answer is…. 2. Lose 5 pounds by Christmas 2024 z Stating SMART Goals … with CVD risk factors in mind What risk factors would you need to manage (reduce/ improve/ increase) to reduce CVD risk?
