HOFFMAN ESTATES FIRE DEPARTMENT STANDARD OPERATING GUIDELINES SOG Category & Identification Number: EFFECTIVE DATE: RESPONSE GUIDELINES – 002 September 1, 2022 NIMS COMPLIANT SOG Title: Code 1/Ambulance Request Revision: 10 APPROVED BY: NUMBER OF PAGES: Re-evaluation Date: Alan Wax January 1, 2026 Fire Chief 4 PURPOSE The purpose of this guideline is to provide standardized and predetermined responses to all incidents classified as an Ambulance Request by Northwest Central Dispatch (NWCD). POLICY It shall be the policy of the Hoffman Estates Fire Department to follow this guideline: however, it may be altered to achieve a more appropriate response based on information received from NWCD or to achieve a faster response based on location and/or status of fire department apparatus and personnel. PROCEDURE A. Response information: For Ambulance Responses: 1. In districts 21, 23, and 24 the district engine and ambulance, or closest available units will respond. On alpha and bravo dispatches the Engine response is at the discretion of the Company Officer. 2. For District 22: When Ambulance 22 is in service the response will be the district Ambulance and Squad 22 or the closest units. On alpha and bravo dispatches the Squad response is at the discretion of the station officers. 1 3. If additional assistance is required, such as in a full arrest or bariatric patient, Tower 22 and Battalion 6 shall be requested to the scene (if not already on scene). If additional assistance is required during transport to the hospital, the personnel should be taken from Tower 22. If the transport is to St Alexius, Tower 22 then follows to the hospital to pick up their member. 4. Interstate 90- Tollway Responses Westbound 56.1 – 59.25 Eng. 24, Amb. 24, & Twr 22 Eastbound 58.5 – 62.5 Eng. 24, Amb. 24, & Twr 22 Westbound 59.25 – 61.8 Twr 22, Amb. 22, & Squad 22 Eastbound 62.5 –66.0 Twr 22, Amb. 22, & Squad 22 Note: The above response designations identify the still district ambulance. Dispatch may follow AVL. 5. If additional assistance is required during transport to the hospital without Tower 22 on the scene, the additional personnel will be taken from the engine or squad company. a. Transport to St Alexius Medical Center: i. If the engine remains at two qualified personnel, they will return to quarters in-service and notify the Shift Commander and NWCD of their status. ii. If there are not two qualified persons remaining, the engine will follow out of service to retrieve the necessary personnel. iii. In District 22, the engine / tower should follow to the hospital and pick up their member before returning to Station 22. b. Transport to any other hospital: i. If the engine / tower remains at two qualified personnel, they will return to quarters in-service and notify the Shift Commander and NWCD of their status. ii. If there are not two qualified persons remaining, the engine / tower will return to quarters Out Of Service and notify the Shift Commander and NWCD of their status. B. Transport Criteria: 1. Transportation to Hospital: All medical emergency transporting of patients will be to St. Alexius Medical Center, our system associate hospital, with the following exceptions: a. Lutheran General Hospital – Level I Trauma and situations involving carbon monoxide poisoning, if incident meets transportation criteria. 2 b. Stroke Centers – Follow NWCH SOP for transport decision tree to Comprehensive or Primary Stroke Center. See Northwest Community Hospital Patient Transport/Selection of receiving Hospital Policy Manual T- 2. (effective 12/01/16) c. Another hospital 1) Under the direction of the Medical Control / ECRN through a telemetry/ cellular phone communication order given by a physician for special transportation to another medical facility. 2) Under the direction of MABAS as part of a mass casualty situation. 3) Closer hospital based upon proximity (time/distance) or due to St. Alexius Medical Center being in a “by-pass” mode. 4) Every effort should be made to transport to St. Alexius, but in certain cases Alexian Brothers Medical Center, Northwest Community Hospital and Sherman Hospital are approved as transport destinations as long as approved by St. Alexius and the patient signs the “Refusal of Recommended Destination” section of the Patient Care Report. The EMS crew should then contact the transport destination hospital. (NWCEMSS Policy T-2) d. Mutual Aid Responses 1) Under the direction of the Medical Control / ECRN through telemetry/cellular phone communication order given by a physician for special transportation to another facility. 2) Under the direction of MABAS as part of a mass casualty situation. 3) To the closest facility normally transported to by the stricken department. e. I90 Tollway 1) Based on patient care criteria and the discretion of the Incident Commander, transport should be to the closest hospital based on travel direction and location on the Tollway. i. Highway Turn-a-rounds are not to be utilized while transporting to the hospital. 2. Response to the scene shall be made utilizing all warning devices including lights, sirens and opticom. 3. Transport from the scene to the hospital. a. On a Basic Life Support call, transport to the hospital will be on a non- emergency basis without use of emergency lights and siren. 3 b. All patients who require Advanced Life Support care and/or meet time-sensitive criteria the patient may be transported from the scene to a hospital emergency department WITH lights, sirens and opticom, unless specifically contraindicated by the patient’s condition. c. For further clarification and policy, see Northwest Community Hospital MICU Policy Manual L-2. (effective 04/01/04) 4. Refusal of transportation to St. Alexius Medical Center: If the patient refuses the transportation, then telemetry/cellular phone communication with the system hospital should be made with documentation of the situation. Paramedics can then transfer responsibility to private ambulance services of equal or greater level of care than the level of care required for the patient’s condition.. DEFINITIONS Code 1/Ambulance Request – Any response of a medical nature including carbon monoxide with symptoms. Basic Life Support (BLS): A basic level of pre-hospital and inter-hospital emergency care, and non-emergency medical care that includes airway management, cardiopulmonary resuscitation (CPR), control of shock and bleeding and splinting of fracture, as outlined in the BLS National Curriculum of the U.S. Dot and any modification to that curriculum specified in rules adopted by IDPH pursuant to the EMS Act. Advanced Life Support (ALS): An advanced level of pre-hospital and inter-hospital emergency care and non-emergency medical care that includes basic life support care, cardiac monitoring, cardiac defibrillation, intravenous therapy, administration of medications, drugs and solution, use of adjunctive medical devices, trauma care and other authorized techniques and procedures, as outlined in the ALS National Curriculum of the U.S. Dot and any modifications to that curriculum specified in the rules adopted by IDPH pursuant to the EMS Act. EXCEPTIONS Where MABAS Illinois Policies and Procedures current online reference or MABAS 1 Operating Guidelines/Policy Statements are in conflict with HEFD Standard Operating Guidelines, HEFD Standard Operating Guidelines will take precedence.
