Bennett DPM, FACFAS College of Podiatric Medicine Des Moines University Course Introduction – Review Syllabus Lecture format Two examinations One assignment – Expectations- Why? Professionalism Boards Objectives: recognize the role of student and physician as a lifelong learner. Recall and recognize the overarching course goals and objectives and how they relate to the improved delivery of healthcare and success as a future clinician. Demonstrate understanding of the many roles a physician plays in the delivery of health care. Course goals: The Health Systems, Community Medicine and Medical Jurisprudence course is designed to introduce the student to aspects of health care beyond basic science and the clinical aspects of patient care. The Jurisprudence section of the course will expose the student to the medico-legal aspects of the practice of health care. The Health Systems and Policy portion introduces fundamental principles and concepts associated with the delivery of health care in today’s ever-changing health environment. The course will also introduce the student to concepts in epidemiology and transmissible diseases. The course will elaborate on concepts in research- based methodology and evidence based medicine from previous courses. Additionally, this course will review ways to enhance patient safety and experience as well as identify the role of various forms of bias that can affect patient care. Reality Medicine is not JUST about science. – The “Practice” of medicine is not always intuitive – It is not enough to just know the answers/diagnosis/treatment The effective practice of medicine and health care delivery involves having a thorough understanding of: – Drivers of patient satisfaction – Outcomes/Quality – Legal implications – Ethical implications – Payment – Cost – Variance Medicine is also a business and the business of medicine is ever changing As you become a physician what are some of the roles and responsibilities you may have? Life-Long Learner Medical/Surgical Treatments – EBM – “State of the art” Learn from mistakes and decrease errors Cost Effective Care High quality decision making based on outcomes and medical necessity Patient as consumer Patient engagement/experience/compliance Health Care regulations- billing, coding, documentation, etc. Impact of health care system on economy Being a lifelong learner is an intentional, Develop a plan to be successful proactive decision by the individual. Set realistic goals of accomplishment Identify the area of need Seek feedback from peers (surround -knowledge based yourself with individuals who are a supportive) -clinical skills Seek evidence -Organizational skills -Communication -time management Key points from the article: “Lifelong Learning at work’ BMJ 2008;336:667-9 § Lifelong learning is a process of continuously scrutinizing and building on your practice to be the best doctor § Always have three questions in your mind: -where am I going -how am I getting on -Where to next § Use electronic tools that provide filtered information at the point of care because you cannot hope to stay up to date on everything § Keep a learning portfolio so that you can see how you have already developed and how you can develop in the future § Keep your learning effort alive by discussing them with your peers and supervisor I just want to “practice” medicine All of the components of this course are a part of the practice of medicine and health care delivery You as the physician are ultimately going to be held responsible for all of the topics discussed in this course – Billers and coders- it is YOU that will be audited and have to face the consequences – Legal team- it is YOU that will be sued or fined. – Administrators- it is YOU that will be held accountable for outcomes and cost containment. – YOUR payment will be based on many of the new “quality” and “outcome” algorithms and the patient PERCEPTION of the care they received. – Errors- as the team leader many times you will be held to the highest standard and be held responsible.
