Details of course; ii. Duration of course; iii. Hours per week; iv. Proof of payment(receipt or invoice no.); 05 Bank Statement of the last 3 months attached with a recent bank certificate (If the student will be sponsored the following documents are required): i. Sponsor’s Bank Statement (last 3 months); ii. Signed Declaration letter from sponsor; iii. Copy of identification of sponsor(front and back); 06 International Bank card corresponding with bank statement provided (Front & Back); 07 Medical and Travel Insurance: MQF level 5 and over; i. Valid for Schengen area; ii. Minimum medical coverage of €30,000; iii. Covering a minimum of 120 consecutive days from first point of entry in Schengen area; iv. Must be valid for study purpose; MQF level 4 and below; i. Valid for Schengen area; ii. Minimum medical coverage of €30,000; iii. Valid for all duration of stay, starting from first point of entry in Schengen area and ends upon exit from Schengen area; iv. Must be valid for study purpose; 1 of 2 08 Full Passport Copy; 09 Flight tickets: i. Full flight itinerary to Malta must be provided indicating students name, date of flight and all stops; ii. Entry ticket must be two weeks max prior to course commencement date; iii. If the course is less than six months, return tickets following the end of course is required; 10 Accommodation (first 15 nights from date of arrival in Malta); i. Must be in applicant’s name; ii. If provided by the school, the school must provide a letter stating name, surname, passport number, address & duration of accommodation; iii. Lease agreement must be signed by both parties and indicate duration, home address and applicant name; iv. Declaration of proof must be stamped and signed by a local lawyer or notary & proof of residence must be provided ex. Electricity bill; v. MTA license must be valid and signed by the Malta Tourism Authority. A fully filled and signed rental declaration form must be provided with the MTA license; vi. Accommodation booking confirmations from online platforms are accepted; 11 In case of minors the following documents are also required: i. Birth certificate (original and translated in English by an official translator); ii. A copy of parents’ passport; iii.
