Z-Track Injection Method
- The Z-track method is utilized for intra-muscular (I.M.) injections, particularly for dark-colored or irritating medications, like iron solutions, haloperidol, or vistaril.
- This technique prevents staining of subcutaneous tissue, minimizing irritation and discomfort.
Key Precautions for Z-Track Injection
- Avoid injecting into skin that appears lumpy, reddened, irritated, bruised, stained, or hardened.
- Add 0.3–0.5 ml of air into the syringe after obtaining the correct medication dosage to create a barrier.
Needle Selection and Technique
- Change the needle post-drawing medication to maintain injection integrity.
- Use a long (2 inches) 21- or 22-gauge needle, depending on the patient's size, to ensure deep muscle placement.
Injection Site Guidelines
- Administer Z-track injections in large muscles, such as gluteus medius or gluteus minimus.
- Aspirate the syringe before injecting to verify that no blood vessel has been punctured; if blood enters the syringe, discard the attempt.
Procedure After Incorrect Injection
- If blood is aspirated, discard the syringe and medication. Start again with a new syringe, medication, and site.
