Urological Symptoms
- Urologic symptoms: can be used to identify the specific characteristics of prostate cancer and select a treatment option
- International Prostate Symptom Score (IPSS): questionnaire that assigns numerical values to the severity of urinary symptoms
- Nocturia: the need to get up at night to urinate; can be caused by various issues such as cardiac problems, excessive fluid intake, or prostatic problems
- Hematuria: presence of blood in the urine; can be visible (gross hematuria) or only detectable microscopically (microscopic hematuria)
- Risk factors for hematuria: male gender, age older than 35, occupational exposure to chemicals/dyes, analgesic abuse, history of gross hematuria, history of urologic disorder, pelvic irradiation, chronic urinary tract infection, exposure to carcinogenic agents, chronic indwelling foreign body
- Three glasses test: used to determine the cause of hematuria
- Hematuria in the first part of micturition: likely a problem with the urethra
- Hematuria throughout micturition: may indicate a problem with the bladder or kidneys
- Hematuria only at the end of micturition: typical of a bladder tumor and the most worrisome case
Urological Pain
- Ureteric pain: pain in the ureter; may occur due to stones in the ureter; may mimic the symptoms of cystitis
- Vesical pain: pain in the bladder; may be caused by a stone obstructing the neck of the bladder
- Scrotal pain: pain in the scrotum; can be caused by different diseases, including testicular torsion or orchiepididymitis
- Testicular torsion: rotation of the testicle that can cause twisting of the artery and temporary ischemia; a medical emergency; requires manual or surgical derotation within 10-12 hours.
Prostate Cancer
- Radical prostatectomy: surgical removal of the prostate; techniques have shifted over time from open retropubic approach to minimally invasive treatments (laparoscopy, robotic surgery)
- Lymph node dissection: surgical removal of lymph nodes; may be necessary in prostate cancer to remove affected nodes and determine if the cancer has spread to the lymph nodes
- Lymph nodes removal: primarily used for staging purposes and may not significantly impact cancer progression
- Importance of mapping the prostate: crucial to have a detailed map to determine if surrounding structures should be preserved or removed during surgery
