The Urinary System
- Includes: Abdominal Aorta, Inferior Vena Cava, Renal Artery, Kidney, Renal Vein, Ureter, Urinary Bladder, and Urethra
Definitions
- Cystitis involves inflammation and/or infection of the bladder
- Pyelonephritis involves inflammation and/or infection of one or both kidneys
- Bacteriuria indicates presence of bacteria in the urine
- Bacteremia indicates presence of bacteria in the blood
- Urosepsis occurs when a severe illness starts in the urinary tract and spreads to the bloodstream
Urinary Tract Infections (UTI's)
- An infection present in some portion of the urinary system
- Commonly a nosocomial infection typically linked to indwelling catheters (CAUTI)
Diagnosis
- Includes a Urinalysis and Urine Culture
- Urinalysis tests include: Leukocyte Esterase, Nitrates and nitrites, White Blood Cells
Risk Factors
- Gender can be a risk factor
- Incontinence can heighten the risk of a UTI
- BPH, or enlarged prostate, poses a risk
- Improper cleaning.
- Incomplete emptying of the bladder
- Indwelling catheters
Common Organisms
- E. Coli is the most common organism in elderly and women
- Klebsiella
- Enerococcus
- Proteus
- Pseudomonas
- Enterobacter
- Candida
Clinical Manifestations, Symptoms of UTI include
- Strong urine odor
- Strong urge to urinate
- Burning with urination
- Cloudy or pink urine
- Blood in urine
- Suprapubic pain
Treatment
- Includes: Antibiotics, fluids, other medications, and pyridium
Prevention
- Reduce the time that an indwelling catheter is needed
- Use sterile technique when inserting
- Prevent backflow or reflux from the catheter system
- Limit opening of the drainage system
- Provide good catheter and perineal care
Obtaining Specimens
- Method depends on if patient is voiding, has a foley catheter
