Urinary System Anatomy

Kidneys:
- Located on the posterior abdominal wall, between T12 and L3 vertebrae.
- Right kidney is positioned slightly lower than the left.
- Hilum: lies at the transpyloric plane, approximately 5 cm from the midline.
- Superior part: left kidney aligns with 11th rib, right kidney with 12th rib.
- Inferior part: extends to the iliac crest.
- Surrounded by renal capsule, perinephric fat, renal fascia, and paranephric fat.
- Internal structure:
- Cortex (external 1/3): contains renal corpuscles, convoluted tubules, collecting tubules, and blood vessels.
- Medulla (internal 2/3): contains renal pyramids, renal papilla, collecting ducts, straight tubules, and loop of Henle.
- Renal columns: extend from the cortex into the medulla, housing blood vessels and anchoring structures.
- Hilum of the kidneys leads to the renal sinus.
- Arterial supply: Abdominal aorta (T12) → renal arteries (L1/L2 IVD) → 5 segmental arteries.
- Venous drainage: Segmental veins → renal veins → inferior vena cava.
- Lymphatic drainage: Lymph vessels follow renal arteries → aortic nodes (para-aortic).
Ureters

Course:
- Originate at the apex of the renal pelvis (L1), descend along the lumbar vertebrae.
- Traverse anterior to the bifurcation of the common iliac arteries, passing near the ischial spine.
- Connect to the posterosuperior angle of the bladder.

Constrictions:
- Pelviureteric junction
- Level of the common iliac arteries
- Entrance into the bladder

Blood supply:
- Ureteric branches from renal, gonadal, abdominal aorta, common iliac, and internal iliac vessels.
- Superior: renal vessels
- Middle: gonadal vessels, abdominal aorta
- Inferior (pelvic): common iliac artery, inferior vesical artery, vesical venous plexus

Lymphatic drainage:
- Superior: aortic/caval lymph nodes
- Middle: common iliac nodes
- Inferior: external/internal iliac lymph nodes
The Bladder

Structure:
- Hollow, muscular organ with strong walls.
- Apex: superior edge of the pubic symphysis.
- Fundus: opposite the apex.
- Body: between the apex and fundus.
- Neck: origin of the urethra.
- Internal: detrusor muscles, internal urethral sphincter, trigone (ureteric and urethral openings), inter-ureteric torus.

Relations:
- Superior surface: peritoneum, small intestine, sigmoid colon.
- Posterior surface: seminal vesicles, ampulla of the ductus deferens, rectovesical septum, rectovesical pouch, rectum, uterus, uterine tubes, uterovesical pouch.
- Inferolateral surfaces: pubic bones, pelvic floor, retropubic space (apex), prostate gland (neck).

Blood supply:
- Superior vesical artery: anterosuperior parts.
- Inferior vesical artery (males): fundus and neck.
- Vaginal artery (females): fundus and neck.

Venous drainage:
- Vesical venous plexus, inferior vesical to internal iliac vein, internal vertebral venous plexus.

Lymphatic drainage:
- Superolateral: external iliac nodes.
- Fundus and neck: internal iliac nodes.
The Urethra

Females:
- Straight tube (approximately 4 cm long).
- Runs from the internal urethral orifice in the bladder, along the anterior wall of the vagina.
- Opens into the vaginal vestibule at the external urethral orifice.

Males:
- Intramural: neck of the bladder, internal urethral sphincter.
- Prostatic: prostate gland, urethral crest, prostatic sinuses (prostatic ducts), seminal colliculus (prostatic utricle, ejaculatory ducts).
- Membranous: inferior prostate gland to the perineal membrane, external urethral sphincter.
- Penile (Spongy): bulb of the penis to the external urethral orifice, corpora spongiosum, bulbourethral glands, navicular fossa.
Innervation

Kidneys & Ureters:
- Sympathetic: Abdominopelvic splanchnic nerves (T11-L2)
- Parasympathetic: Vagus nerve (CN X)

Bladder:
- Sympathetic: Hypogastric plexus (T11-L2/L3).
- Inhibits detrusor muscle (relaxes) – expansion.
- Contracts internal urethral sphincter.
- Pain from superior surface.
- Parasympathetic: Pelvic splanchnic nerves (S2-S4).
- Motor to detrusor muscle, contracts bladder.
- Relaxes internal urethral sphincter.
- Afferent (reflex and pain) fibres.
- Sympathetic: Hypogastric plexus (T11-L2/L3).

Urethra:
- Females: Vesical nerve plexus and pudendal nerve (mixed sympathetic, parasympathetic, and visceral afferents).
- Males: Prostatic nerve plexus (mixed sympathetic, parasympathetic, and visceral afferents).
- Males: Dorsal nerve of the penis.
