Reporting Failures to Respond
- Failures to respond to incidents must be documented and reported through proper channels.
- Situations that may cause a failure to respond include mechanical breakdown, injury to personnel, failed communication, and collisions preventing response.
Immediate Reporting
- Failures to respond must be immediately reported to the TFS Communications Centre via radio or telephone to allow for replacement apparatus to be dispatched.
Notification and Investigation
- The Company Officer must notify the respective District Chief via telephone.
- The District Chief will investigate all failures to respond and implement/recommend actions to prevent recurrence.
Serious Failures to Respond
- The District Chief must immediately advise the Platoon Chief of failures to respond that result in serious consequences, potential liability, injury to personnel, or damage to apparatus/equipment.
Reporting Failures to Respond
- The Company Officer must submit an e-mail report to the District Chief (with a copy to the Platoon Chief) including:
- Incident Number
- Explanation of the failure to respond
- Company Officer's name and personnel number
Follow-up Responsibility
- The Platoon Chief is responsible for advising the Division Commander as necessary.
