Study Notes
Unit 2h: Intro to Risk Assessment
-
Lecture Objectives:
- Identify risk assessment aims and objectives, differentiating qualitative and quantitative assessments.
- Review hazard control hierarchy and its role in risk assessment.
- List risk assessment tools, outlining procedures for job safety analysis (JSA).
- Define incidents, explaining procedures and legal requirements for investigating them.
- Classify emergency types and outline factors for successful evacuation/response plans.
What is Risk Assessment?
- A risk assessment is a systematic examination of workplace hazards to prevent harm to individuals.
- It's crucial to protect workers and others, prevent accidents/illnesses, and avoid legal issues (e.g., court cases, insurance increases).
- Accidents and poor health can severely impact individuals and businesses (lost output, damaged machinery, financial losses).
How to Assess Workplace Risks
- Follow these five steps:
- Identify hazards.
- Determine who might be harmed and how.
- Evaluate risks and identify precautions.
- Record findings and implement controls.
- Review and update assessments as needed.
- Avoid overcomplicating the process. Many risks are already known.
Hazard Analysis Methods
-
Job Safety Analysis (JSA) or Job Hazard Analysis (JHA):
- Document hazards, causes, and control measures.
- Important for complex tasks like equipment maintenance, crane operations.
-
Process Hazard Analysis:
- A regulatory requirement preventing incidents like the Bhopal Disaster.
- Analyzes hazards associated with processes involving hazardous chemicals.
Hazard Severity and Probability
-
Severity is categorized into major, serious, and slight.
-
Probability is categorized into high, medium, and low.
-
Risk = Severity x Probability
-
Examples of high risk levels (6-9), moderate (3-5), and low levels(1-2).
-
HAZOP (Hazard and Operability Study), JSA, Failure Mode and Effects Analysis (FMEA), and Fault Tree Analysis are common analysis tools.
Process Hazard Analysis (HAZOP)
-Analyze potential hazardous occurrences in chemical processes.
- Identifies and analyzes hazards in highly hazardous chemicals (HHCs)
- Evaluates each process in the establishment to identify, evaluate, and control hazards.
Checklist Analysis of Process
- Questions used for evaluating risks within a process.
Accident Investigation
-
What is an Accident? An unplanned event that causes harm or damages property, but can also be a "near miss" or "dangerous occurrence."
-
Importance of Investigating Accidents: To identify the root cause, prevent future incidents, comply with regulations, and determine costs.
-
Steps to Investigate Accidents: Report the incident to the appropriate person. Provide first aid and medical attention. Identify and investigate cause. Develop a solution and implement corrective actions. Evaluate the effectiveness of corrective action.
