Study Notes
Type 2 Diabetes
- Results from abnormal insulin secretion, or resistance to insulin action in target tissues
- Number of insulin receptors in the islets of Langerhans decreases slightly with age, leading to reduced sensitivity to high glucose levels and delayed insulin production
- In some older adults, the liver fails to inhibit glucose production after insulin binds to the receptor
Symptoms of Hyperthyroidism
- Integumentary: warm, flushed skin, fine, soft hair
- Neurologic: difficulty concentrating, excitability or nervousness, tremor, difficulty sleeping
- Cardiovascular: tachycardia, full, bounding pulse, hypertension
- Respiratory: dyspnea on exertion and at rest
- Genitourinary: gynecomastia in males, weakness, fatigue, muscle atrophy
- Eyes: exophthalmos, inflammation of the conjunctivae, cornea, or eye muscles
- Potential complications: myxedema coma, thyroid storm, angina, heart failure
Elder Abuse
- Risk factors: caregiver stress, negative view of aging
- Interventions:
- Evaluate potential for harm and consult social work if necessary
- Monitor patient for signs of neglect or abuse
- Discuss abusive relationship with patient to assist in recognition of the problem
- Contact social services for financial and legal support and alternate living arrangements
- Refer patient to Adult Protective Services or other community agencies that deal with elder abuse
Age-related Chronic Illnesses and Medications
- Chronic pain and emotional factors may contribute to fear related to sexual performance
- Interventions:
- Promote a nonjudgmental environment with reduced anxiety and fear
- Encourage patient to verbalize feelings about performance
- Offer resources such as a counselor or other mental health professionals
- Encourage patient to focus on less threatening forms of intimacy to promote positive reinforcement
