Study Notes

Trichomonadida Trichomonas

  • No cyst stage is present
  • This species exists only as a trophozoite form

Trichomonas Species

  • T. vaginalis
  • T. hominis
  • T. tenax
  • Locations include oral, vaginal, prostatic, and intestinal tracts

Trichomonas Species Pathogenicity

  • T. tenax: Found in oral gingival and tracheobronchial sites, considered nonpathogenic
  • Pentatrichomonas hominis: Found in the intestinal tract, considered nonpathogenic
  • T. foetus: Causes bovine trichomoniasis, is invasive to the fetus (placenta, fetal lung, gut, lymph nodes), and causes abortions in infected cattle
  • Prevalence of vaginal trichomoniasis in some African populations is between 11-25%

Trichomoniasis

  • STD spread through sexual intercourse
  • Most common infection site is the vagina in women and the urethra (urine canal) in men
  • Incubation period is 4 to 4 weeks

Laboratory Diagnosis

  • Microscopic examination of vaginal or urethral discharge for jerky motility and characteristic shape
    • Saline wet mount preparation
    • Trophozoites can be found in urine or prostatic secretions in males
    • Staining with acridine orange, papanicolaou, or giemsa stains
    • Direct fluorescent antibody (DFA) is more sensitive
  • Culture is recommended if direct microscopy is negative and is considered the gold standard (95% sensitive)

Trichomoniasis Symptoms (Females)

  • Abundant, frothy vaginal discharge ranging in color from gray to green to yellow, watery or milky consistency
  • Foul odor
  • Itching around the vagina
  • Pain during sex and urination

Trichomoniasis Symptoms (Males)

  • Mild urethral itching
  • Pain during urination
  • Pain during sex
  • Prostate gland inflammation

Trichomonas Vaginalis Discovery

  • Little information is described about the discovery of Trichomonas vaginalis between 1934 and 1939 by Procaccini

Treatement

  • Metronidazole (Furazol)
  • Tinidazole (Fasigyn)
  • Secnidazole (Flagentyl)
  • Both sexual partners should be treated at the same time, even if one partner shows no symptoms
