Study Notes

Trip Information

  • Trips can be booked, changed, canceled, etc.
  • Method of transportation: VAN, BUS, MRB, PAR, etc.
  • Date of appointment and day of the week
  • Time of appointment
  • Round trip or one-way
  • Pickup location and address
  • Best contact number for the member
  • Destination location and address
  • Facility phone number
  • Destination suite/facility name/doctor name

Trip Remarks

  • Required for internal use, specific to each trip
  • Must include time, time zone, and important trip details
  • Notes may include changes made to a trip, overrides, or problems encountered
  • Remarks should be added when a trip is opened after intake, or if a call is transferred
  • Includes any information that needs to be known by another MSR or leadership regarding the trip

Member Profile Notes

  • For internal use, notes are profile-wide
  • Accessed through the "View Member" page
  • Notes should follow a template and be marked as a Red Flag with No Expiration
  • Wheelchair notes include member height, weight, and transfer abilities
  • Examples of templates include standard manual wheelchair, bariatric manual wheelchair, standard electric wheelchair, and bariatric electric wheelchair

Trans Prov Notes

  • For the driver, includes information such as:
    • Member is bringing an item (cane, crutches, car seat, service animal)
    • Member is bringing additional passenger(s) (escort, child, caregiver)
    • Instructions for pick-up (gate code, landmarks, clothing)
  • Does not include HIPAA information (home address, phone number)

Additional Services

  • Additional stops can be added, including pickup, destination, additional, and return stops
  • Stop types are categorized using the key: P = Pickup, T = Destination, A = Additional, R = Return
  • Members can search for previous addresses or enter new addresses, which will auto-populate remaining information
  • Facility details, including phone number and suite/facility name, can be added and saved.
