Transition Procedure
- Transition procedure aims for a smooth, compliant exit and transition of families and children
- Includes knowledge sharing of IFSP under IDEA Part C and IEP under Part B early childhood preschool programs, as well as other community programs.
Informing All Families About Transition
- Families can remain enrolled until the child no longer needs services, reaches age 3, or the family chooses EIS over 3 and starts preschool under an IEP.
- Children can start preschool under IEP at the start of the school year or later, due to late referral (referred between 97 and 46 days before their third birthday).
- Transition plans are documented within IFSPs. Plans include transition activities and timeframes, and community resources for those not referred to LEA.
Inclusion of LEA in Transition Planning with Formal Referrals
- Children enrolled in or continuing to receive support over 2.5 years old may be eligible for early childhood special education.
- Refers families to support programs within their town.
Referral Forms and Information Sharing
- Monthly milestone reports are emailed with upcoming transition dates/reminders.
