Study Notes

Clinical Manifestations of Transient Myeloproliferative Disorder (TAM)

  • Can be observed in other disorders like trisomy 15 and 13, leukemias, inflammatory processes, chemotherapy.
  • Possible symptoms: ascites, pericardial or pleural effusions, hepatic fibrosis, disseminated intravascular coagulopathy (DIC), renal, hepatic or cardiopulmonary failure, and hydrops fetalis.

Laboratory Findings of TAM

  • Presence of megakaryoblasts in peripheral blood.
  • Thrombocytopenia is the most common finding.
  • Moderate leukocytosis with myeloid left shift.
  • Basophilia is present.
  • Coagulopathy, specifically DIC (10%).
  • Elevated conjugated bilirubin, secondary to liver involvement.
  • Bone marrow examination is not necessary but often done to rule out leukemia.

Prognostic Factors of TAM

  • Poor prognostic factors for early death include:
    • High WBC count.
    • Increased bilirubin.
    • Elevated liver function tests (LFTs).
    • Failure to normalize blood counts.

Immunophenotyping Study of TAM

  • Immunophenotyping study using multiparameter flow cytometry in TAM and Acute Myeloid Leukemia (AML) in Down syndrome patients suggests that the blast population shares similar characteristics:
  • Positive reaction for CD45, CD33, CD41, CD61, CD34.
  • Negative reaction for CD14 and CD64 antigens, indicating megakaryocytic differentiation.
