Purpose and Responsibility

  • The purpose of this policy is to provide guidelines for reporting incidents, accidents, injuries, and illnesses in Toronto Fire Services (TFS).
  • All TFS personnel are responsible for following worker injury occurrence procedures.
  • District Chiefs are responsible for ensuring that TFS personnel follow these procedures.

Definitions

  • Assess: The immediate Supervisor and worker assess the need for external treatment.
  • Incident Only: An accident/incident not requiring first aid, medical aid, or lost time from work.
  • First Aid Only: A minor injury treated with first aid at the workplace, with no services from a healthcare professional.
  • Medical Aid Only: Injury requiring services from a healthcare professional, but no lost time beyond the day of the accident.
  • Lost Time: An accident/incident requiring attention from a healthcare professional, resulting in lost time beyond the next regularly scheduled shift.
  • Report: All work-related injuries and complaints are reported promptly to the immediate Supervisor.
  • Return-to-Work: The employee returns to regular or modified duties, providing the Supervisor with the WSIB Form 8 or TFS Return-to-Work (RTW) Form.

Scope

  • For the purpose of this policy, a “Supervisor” means a person in charge of a workplace or with authority over a worker.

Incident and Accident Reporting

  • All accidents, incidents, whether resulting in lost time or not, are reported using the Supervisor’s Report of Injury/Incident.
  • The Supervisor completes the report, conducts a detailed investigation, gathers witness statements, and submits the report to the District Chief.
  • The District Chief enters the report into Quatro Safety for authorization by the Division Chief/Platoon Chief.
  • Accident reports are used to identify causes of injuries or illnesses, which are grouped into people, equipment, environment, and procedures.
  • An online tool is available for Supervisors to identify hazards during investigations.
  • The Supervisor’s Report of Injury/Incident must be clear and concise, as it is used for legally required notifications to the WSIB and Joint Health & Safety Committees.
