Questions and Answers

A patient with a cast on their lower leg reports a persistent, severe pain that is not relieved by prescribed analgesics. Which of the '7 Ps' of orthopedic assessment is MOST directly represented by this situation?

  • Pallor
  • Pain (correct)
  • Puffiness/Pressure
  • Paresthesia

In the context of immobilization and its effects on the body, which of the following BEST exemplifies a systemic effect?

  • Decreased range of motion in the immobilized joint.
  • Loss of muscle mass in the affected limb.
  • Cardiovascular deconditioning due to reduced physical activity. (correct)
  • Development of a pressure injury under a cast.

A patient who recently underwent abdominal surgery is being discharged. The physician orders an abdominal binder. What is the PRIMARY purpose of this intervention?

  • To support weakened abdominal muscles. (correct)
  • To reduce the risk of post-operative infection.
  • To promote circulation to the abdominal area.
  • To limit patient mobility and prevent falls.

Following an acute ankle sprain, a patient is instructed to use the RICE protocol. What does the 'I' in RICE primarily aim to achieve?

Flashcards

The 7 "P"s of Neurovascular Assessment

Pain, Pallor, Paresthesia, Pulselessness, Polar Temperature, Paralysis, Puffiness/Pressure.

Systemic vs. Local

Systemic: Affecting the entire body or multiple systems. Local: Affecting a specific area or part.

Types of Immobilization Devices

Splints, casts, slings, binders, traction, external fixators.

Use of Abdominal Binder

To support abdominal muscles weakened by surgery, pregnancies, or inactivity.

Study Notes

  • There are 7 "P"s to assess in a patient

The 7 "P"s

  • Pain: Pain is not relieved by medication or is disproportionate to the injury
  • Pallor: Skin appears pale, white, or blanched and may have a waxy look; capillary refill is absent
  • Paresthesia: Abnormal sensations such as "pins and needles" or numbness
  • Pulselessness: Absence of pulses in the extremity (a late symptom)
  • Polar Temperature: The limb is cold to the touch (a late symptom)
  • Paralysis: Patient is unable to move the limb (a late symptom)
  • Puffiness/Pressure: Presence of edema and swelling

Systemic vs. Local Effects

  • Systemic effects involve the entire body system, including cardiovascular, respiratory, GI tract, musculoskeletal, integumentary, and psychological aspects
  • Local effects include pressure injuries, impaired joint function, and loss of bone or muscle mass in the affected limb

Immobilization Devices

  • Splints
  • Casts
  • Slings
  • Binders
  • Traction
  • External Fixators

Abdominal Binders

  • Abdominal binders support abdominal muscles weakened by surgery, repeated pregnancies, or inactivity

Limb Temperature

  • A cold and clammy limb is an abnormal finding

Heat Application

  • Heat application is commonly used for musculoskeletal problems like joint stiffness, arthritis, contractures, lower back pain, and sprains
  • Heat causes blood vessels to dilate, increasing blood flow

Cold Application

  • Cold application is used for fractures, sprains, to prevent swelling, and to cool the body during a fever
  • Cold causes blood vessels to constrict, decreasing blood flow

RICE Acronym

  • RICE stands for Rest, Ice, Compression, and Elevation
