    Study Notes

    Testicular Disorders

    • Scrotal pain can be a symptom of several pathologic conditions. It can be sharp, dull, aching, uncomfortable or tender.
    • Testicular pain can be an incidental finding or an acute presentation.
    • It is important to determine the cause of testicular pain to evaluate the need for emergent referral and to exclude potentially life-threatening or fertility-threatening conditions.
    • Immediate referral to the Emergency Department (ED) is warranted for sudden onset of severe pain in or swelling of the scrotum, elevation or abnormally positioning of a testicle, scrotal or testicular erythema, and tender scrotal masses.
    • Associated symptoms of testicular torsion include dysuria, abdominal pain, nausea, and fever.
    • Testicular torsion can be caused by trauma, like burns, blunt force, or penetrating injury. Half of testicular torsion cases stem from sporting activities.
    • Testicular cancer is the most common form of cancer in men ages 15-35 years.
    • Factors associated with testicular cancer include scrotal trauma, atrophy, undescended testicles, exogenous estrogen exposure, and family history of testicular cancer.
    • Testicular cancer is typically found on self-examination, presents as a gradually progressive palpable testicular mass, and may be painless or painful.
    • Elephantiasis is caused by filariasis, a parasitic roundworm disease transmitted by flies and mosquitos.
    • Elephantiasis affects the scrotum, causing massive scrotal lymphedema.
    • Epididymitis is the most common cause of acute scrotal pain in men.
    • Chlamydia and gonorrhea are the two most common causes of epididymitis in men under 35 years old, while E. coli is common in men older than 35 years old.
    • Orchitis is a viral or bacterial systemic, bloodborne infection that results in acute inflammation of one or both testicles.
    • Orchitis can be a consequence of mumps, coexist with prostate infections/epididymitis, or be a complication from syphilis, mycobacterial, or fungal infections.
    • Spermatocele is a benign, painless sperm-filled cyst of the epididymis.
    • A hydrocele is the accumulation of fluid within the tunica vaginalis surrounding the testicle.
    • Hydrocele can occur at birth and close at age 1-2 years.
    • The most common cause of painless scrotal swelling is a hydrocele.
    • A hematocele is a collection of blood in the tunica vaginalis surrounding the testicles.
    • A hematocele is usually precipitated from trauma.
    • The differential diagnosis for any testicular disorder or acute scrotal mass/pain should first exclude the possibility of a testicular tumor.
    • The presence of a testicular mass is suggestive of a tumor and indicates the need for immediate referral.
    • Varicocele is a condition where there is an abnormal dilation of the plexus and spermatic veins in the spermatic cord, leading to testicular damage and male infertility.
    • Varicocele is more distinguishable because it resembles a bag of worms on palpation.
    • Testicular cancer often presents with a hydrocele.
    • For a scrotal exam, it's essential to inspect and palpate the skin of the scrotum, epididymis, and spermatic cord for smoothness and lack of tenderness, edema, and discoloration.
    • Diagnostic testing for testicular disorders includes urinalysis/urine culture, ultrasound (US), CT scans, and tumor markers.
    • On exams, ask all male patients about changes in testicular size or the presence of nodules/masses, pain, or penile discharge.
    • Use OLDCARTS (Onset, Location, Duration, Characteristics, Aggravating/Alleviating Factors, Radiation/Relieving Factors, Timing, and Severity) when assessing any discomfort or pain.
