Study Notes
Temporomandibular Joint Anatomy
- The temporomandibular joint connects the mandible (lower jaw) to the cranium (skull).
- The condylar head of the mandible rotates on the underside of the disc within the joint.
- This rotation can occur on multiple axes.
Occlusion Rims and Protrusive Excursion
- The occlusion rims establish the proper bite and should have a 3mm clearance during protrusive excursion (moving the jaw forward).
- The protrusive relationship determines the angle of the horizontal condylar path.
Incisal Angle and Overjet/Overbite
- Overjet (horizontal distance between upper and lower incisors) is inversely proportional to the incisal angle.
- Overbite (vertical overlap of upper incisors over lower incisors) is directly proportional to the incisal angle.
Mandibular Movements (Positions)
- Retrudedcentric Position: This is the fully retracted position of the mandible.
- Right Lateral Position: This is the position when the mandible is shifted to the right.
- Left Lateral Position: This is the position when the mandible is shifted to the left.
- Protrusive Position: This is the position when the mandible is moved forward.
Pantographic Tracings
- Pantographic tracings record lateral and protrusive mandibular movements.
- A pantograph (a fully adjustable articulator) is used to create these tracings.
Working Condyle Movement (Laterotrusion)
- The working condyle rotates around a vertical axis during laterotrusion.
- At the end of laterotrusion, a lateral bodily shift occurs, known as Body Side Shift or Bennett's Movement.
Balancing Condyle Movement (Mediotrusive)
- The balancing condyle moves forward, downward, and inward (medially) during mediotrusive movement.
- The balancing condyle movement involves three axes:
- Horizontal-sagittal (transverse or retruded condyle axis)
- Horizontal-coronal axis (sagittal axis)
- Vertical axis with horizontal rotation
- The movement of the balancing condyle facilitates the descent of the balancing condyle.
