Skin Cleanser - Rinse Gentle Rain
- Intended to cleanse skin and hair.
- Use in the shower/bath, ensures complete removal.
- Not for bedside bathing, do not dilute.
- Apply onto moist cloth/skin, wash, rinse, and pat dry.
- For hair cleansing, lather and rinse well
- Contains fragrance.
Skin Cleanser - No Rinse Remedy Foam Cleanser
- Cleanses skin/ hair.
- Use for bedside bathing.
- Apply 1-3 pumps from palm onto cloth/skin, wash, and pat dry, do not rinse.
- For hair, work cleanser into strands and towel dry, do not rinse.
Moisturizer Remedy Nourishing Cream
- Designed to maintain skin health.
- Helps prevent skin tears; may skin tears up to 50%.
- Use for dry skin treatment
- Do not use for IAD prevention.
- Apply a thin layer to clean skin.
- For feet, avoid between toes and wear socks/shoes to prevent falls.
- To maintain skin health, apply daily after shower/bath while skin is still moist.
- To prevent skin tears, apply twice per day.
- To treat dry skin, apply twice per day.
Protectant Silicone 24% Remedy Hydraguard Cream
- Intended for preventing and treating Mild-Moderate IAD.
- Treats cracked skin.
- Acts as a barrier around wounds.
- Apply a thin layer to the area
- If for wound/tube/drain, allow drying for 1-2 min before dressing, do not use with silicone dressings.
- Avoid between toes and wear socks/shoes to prevent falls.
- For IAD, use after every incontinence episode.
- To treat dry skin, apply twice per day.
- For Peri-wound/tube/drain MASD, apply as ordered.
Protectant Zinc 30% Secura Extra Protective Cream (EPC)
- For Moderate-Severe IAD.
- Prevents IAD in severe diarrhea from C. difficile or gastro-intestinal issues.
- Apply cream onto palm
- Apply in one direction - cream will "follow your hand", do not rub.
- Apply a thick layer (5mm) to ensure the skin is not visible.
- Repeat until the affected area is covered.
- To cleanse during an incontinence episode:
- Use No-Rinse Foam Cleanser on a cloth to clean soiled cream, reapply cream as needed, do not remove all EPC with every incontinence episode.
- To treat Moderate-Severe IAD and prevent IAD apply after every episode.
- To remove all EPC for skin assessment Q3days/prn
- Apply No-Rinse Foam cleanser directly onto the EPC.
- Let sit for 2-3 minutes then wipe with a cloth.
- Repeat until all cream is removed.
Advanced Moisturizer Urea 10% AHA 4% Atrac-Tain Cream
- Treats extremely dry skin.
- Good for lower legs/feet/clients with diabetes or kidney failure.
- AHA removes flakey skin allowing Urea moisturizer to absorb.
- Not a basic skin moisturizer unless specifically ordered.
- Ensure care plan in place, developed by NSWOC/RN/RPN as per PISheet.
- Cleanse with cleanser to remove loose skin, pat dry but leave slightly damp, spread cream and rub in gently.
- Caution, do not apply between toes
- May sting if applied to broken skin.
- Test on small area first.
- Apply twice per day, stop when dead skin thins, switch to basic moisturizer.
Skin Fold Gauze - PHMB AMD/PHMB Gauze
- Intended to prevent & provides intertrigo relief
- Use for moisture in skin folds/finger/toe web spaces/hands with contractures.
- Cleanse with cleanser and pat dry.
- Cut to ensure full coverage to the base of the fold.
- Weave toe/finger gauze; do not completely wrap digit or cut off circulation.
- Caution, do not use with creams, powders, sprays, polysporin, petrolatum-based products or antifungals like Clotrimazole.
- Change daily or when soiled/moist.
Skin Fold Textile/Cloth - Silver InterDry Ag Sheet
- Silver is used to treat intertrigo & moisture in skin folds, finger/toe webs spaces and hands with contractures where the client risks local infection or has infection symptoms.
- Cut lengthwise or width-wise
- For Day 1
- Cleanse and pat dry, lay sheet into the fold, smooth down, ensure 5cm is outside the skin fold.
- Write date on exposed sheet.
- For Days 2, 3, 4, 5
- Cleanse daily, reposition sheet.
- Caution, do not use with creams, powders, sprays, polysporin, petrolatum-based products or antifungals like Clotrimazole.
- Assess daily, change sheet on Day 5 or earlier if soiled, some discolouration is expected.
Hydrophilic Wound Dressing Triad
- A wound treatment plan must be in place.
- Triad is a moisture dressing that adheres to weeping areas and it dries providing cover.
- Nurses must provide the initial application.
Caution, not a barrier cream.
- Apply 3mm thick layer so skin is not visible.
- Allow dressing to dry for 15-20 mins.
- Cover dressing if patient unable to be off of wound during drying.
- Triad will just wipe off if it's not allowed to dry or is not protected.
- Change as per care plan. To remove, apply cleanser and wait 2 - 3 minutes and wipe with cloth.
