Salivary Gland Tumors

Benign Tumors (Adenomas):
- Pleomorphic Adenoma (Mixed Tumor): The most common benign tumor, characterized by a mixed epithelial and mesenchymal tissue origin. Characterized by slow growth, well-defined capsule, painless swelling, and a rubbery consistency. Often found in the parotid gland.
- Warthin Tumor (Adenolymphoma): The second most common benign tumor. Usually found in the parotid gland, often bilateral and presents slow-growing, painless, swelling. Originating from epithelial tissue with lymph node involvement. Associated with smoking.
- Canalicular Adenoma: Common in the palate region.
- Basal Cell Adenoma: Occurs in the parotid gland.
- Oncocytoma: Eosinophilic cells, commonly in the parotid gland.
Malignant Tumors (Carcinomas)
- Mucoepidermoid Carcinoma: The most common malignant salivary gland tumor in the major salivary glands. Often found in parotid glands. It is composed of three cell types; mucous, squamous, and intermediate cells. It can present with signs of malignancy, showing ulceration, necrosis, and invasion.
- Adenoid Cystic Carcinoma: The second most common malignant salivary gland tumor. Commonly occurs in the minor salivary glands. Characterized by three main patterns, and is potentially the most dangerous.
- Acinic Cell Carcinoma: A rare malignancy frequently found in the parotid gland. Characterized by large cells with granular basophilic cytoplasm arranged in acinar patterns.
- Carcinoma ex Pleomorphic Adenoma: A malignant tumor that develops within a pre-existing pleomorphic adenoma, often after a long duration (10-15 years).
- Polymorphous Low-Grade Adenocarcinoma: A rare tumor, characterized by cytologically benign cells arranged in various patterns (tubular, lobular, papillary,etc.). It can present in the palate.
General Features of Salivary Gland Tumors
- Clinical Features: Many benign and malignant tumors present as slow-growing swellings, but some, like adenoid cystic carcinoma, may cause pain, ulceration, facial paralysis, and paresthesia.
- Histology: The microscopic examination of tissue samples is crucial. Histological features are employed to differentiate benign from malignant tumors. Different types of carcinoma, squamous cell, adenoid cystic, mucoepidermoid carcinoma, and others may be adjacent to pleomorphic adenoma, adenoCA, or undifferentiated CA.
- Imaging: Imaging studies (CT scans or MRI) are employed to evaluate the size, location, and extent of the tumor.
- Prognosis: Prognosis for salivary gland tumors varies greatly depending on the tumor type, its stage, and response to treatment.
