Study Notes

Safety Surveillance Staff

  • The Safety Surveillance Staff (SSS) is assigned by the STL and has top priority over all other unit assignments.
  • SSS is the ONLY assignment for this PNA.
  • The STL is responsible for making assignments and ensuring break times.
  • Staff pulled to work on another program will not be assigned to be SSS on that program, unless approved by the BSSH Shift Coordinator (RN).
  • All shifts on all units will have one SSS assigned to each wing of the building, they wear yellow vests.
  • Each shift has a four-hour duty for the SSS (except for the night shift who will be assigned SSS duty for 2-hour increments).
  • The Pine unit is the only unit where staff pulled from another unit can be utilized without first obtaining permission from the Shift Coordinator.
  • All shifts on the Pine unit will have one SSS assigned in each of the following areas: male dorm/dayroom area (yellow vest), female dorm/dayroom area (yellow vest) and one SSS assigned to each hallway (orange vest) as a floater.
  • If there is only one patient to one corridor/hallway (east or west), a floater is NOT necessary for that corridor/hallway, and the SSS yellow vest will assume the floater duties.
  • The SSS's main responsibility is to escort patients to and from the restroom, but they may also be utilized for other duties such as medication line monitoring, fresh air breaks, bathing supervision, etc.
  • If the Pine unit only has 2 patients and both patients can be visualized, the SSS will be allowed to sit while observing.
  • Each dorm/dayroom SSS will wear a yellow vest that identifies him/her as the SSS.
  • The hallway floater SSS will wear a bright orange vest that identifies him/her as the hallway floater SSS.
  • The dorm/dayroom SSS duties include:
    • Completing hourly rounds to document patient progress.
    • Actively engaging with patients ensuring they are aware of activities and support options available.
    • Ensuring patient safety at all times by being vigilant for any signs of distress, falls, or other incidents.
    • Maintaining a clean and safe environment by reporting any necessary repairs or maintenance issues.
    • Liaising with unit nursing staff on patient needs.
  • The hallway floater SSS duties include:
    • Checking to ensure hallway shower rooms and restrooms are always locked unless they are in use.
    • If a patient needs the restroom/shower room, the hallway floater SSS unlocks the door and continuously monitors the patient.
    • Prior to exiting and locking the door, a thorough check of the shower room or restroom will be completed to ensure there are no remaining patients in the area.
    • Encourage all patients to stay as active as possible during the daytime hours attending on unit ATD activities, interacting with peers and staff and utilizing the "mini dayroom" and large dayroom as appropriate.

Security and Safety Staff (SSS)

  • Any changes in a patient’s behavior, threats or attempts to engage in suicidal or aggressive behaviors, or any other concerns the SSS have are to be reported immediately to their supervisor via SSS radio.
  • SSS are required to complete a room check sheet, which is to include specific codes to mark down specific locations, transfer status, and reasons for being absent.
  • The SSS Room Check Sheets include information that pertains to buildings 506, 532, 533, 533 North, and 540 at the facility.

Documentation

  • Every time a new document is revised Thomas Stevens, BSN, RN, Assistant Chief Nurse Executive I, is to make a note of the date revised on an approved form.

Buildings

  • Building 506 is the downstairs location of the facility
  • Building 532 is the north and south location of the facility
  • Building 533 is the south location of the facility
  • Building 533 North is the west side location of the facility
  • Building 540 is the Red Oak south side location of the facility
