- The purpose of this document is to establish instructions for operating the Benchmark Ultra Automated Stainer (HIS).
- The Benchmark Ultra Automated Stainer is a machine that is used for immunohistochemistry.
- The Benchmark Ultra Automated Stainer is used for specimens that have been issue on a glass slide.
- The Benchmark Ultra Automated Stainer requires the following equipment: a Ventana/Roche Benchmark Ultra Slide Stainer, antibodies, probes (example of IHC/ISH stain form- Appendix 1), probes, amplifier, protease, bluing reagent and hematoxylin, and hematoxylin II, OD. Detection Kits- ISH Blue, Ultra DAB, Ultra Red, Optiview DAB, Bulk reagents Ultra LCS, Ultra CC1, Ultra CC2, EZ PREP, SSC, Reaction Buffer, Prep and Option kits, Control tissue blocks and slides, Positive charged slides, coverslips, and mounting media, Xylene, 100% alcohol, 95% alcohol, Dawn liquid soap, Lysol, and personal protective equipment (PPE).
- Preventative maintenance is performed annually by Ventana/Roche. Service records are kept in the Immunohistochemistry section.
- Quality control is performed and documented using the Consolidated Completed Staining Run form.
- If immunohistochemistry cannot be performed at this site, tissue sections are cut on charged slides and sent to pathology.
- A copy of the pathology report or a copy of the SF-515 tissue exam request form is obtained.
- Slides, report, and or blocks are taken to Pathology Records Section 447-6.
- Notification is made to the reference lab if there is a large volume of immunohistochemistry requests.
- Immunohistochemistry staining at BAMC have bulk reagents that are concentrates with the exception of Ultra Liquid Coverslip, and Cell Conditioning 1 and 2.
- Each Ventana/Roche Benchmark Ultra is placed as follows: Bottle 1: EZ Prep Concentrate (EZ Prep), Bottle 2: Ultra Liquid Coverslip (Ultra LCS), Bottle 3: Sodium Chloride Sodium Citrate Solution (SSC), Bottle 4: 2 containers labeled #4, both contain Reaction Buffer, Bottle 5: Ultra Cell Conditioning 1 (CC1), Bottle 6: Ultra Cell Conditioning 2 (CC2), if CC2 is used, the bulk container with Reaction Buffer must be decontaminated with dilute Lysol I.C. for 15-30 minutes and rinsed with distilled water at least 5 times, Bottle 7: labeled “OPTION” contains Reaction Buffer.
- Waste containers should be full and waste is disposed as needed in waste containers labeled “Aqueous waste”.
- To start up the Ventana Benchmark Ultra, enter your password into the computer and click on the instrument to be used.
- The Benchmark Ultra can be used to stain slides for histology.
- Slides can be placed in any slide drawer, and the machine will automatically start up.
- If quality of the staining run is not acceptable, the pathologist should be notified and the slides should be re-stained.
- If tissue does not adhere to the slide, one of two things may be the cause: either the tissue was not placed on an uncharged slide, or the tissue was "double dipped". In the case of "double dipping", the pre-cut control was placed on the slide, which caused the entire slide to be placed in the water bath to pick up the tissue.
- The Benchmark Ultra is a machine that is used to stain tissue for immunohistochemistry.
- The Benchmark Ultra must be operated by wearing proper PPE when handling antibodies and reagents.
- When an instrument appears to be leaking, turn it off by opening the left waste door and pressing the button down and unplug that instrument.
- Prepare SSC by mixing 2 bottles of concentrate (2L) with 18L of distilled water.
- Prepare EZ Prep by mixing 1 bottle of concentrate (2L) with 18L of distilled water.
- The Benchmark Ultra must be operated with new, verified control tissues before reporting patient results.
- Prepare SSC and EZ Prep by mixing 2 bottles of concentrate (2L) with 18L of distilled water and 1 bottle of concentrate (2L) with 18L of distilled water, respectively.
