    • Dental implants offer a way to replace teeth that have been lost and preserve surrounding oral tissues.
    • Bone classification is important in the planning and execution of dental implants, as the density of bone in a potential implant site determines factors such as: time frame for integration of the implant, window of time for prosthetic loading, and success of osseointegration.
    • When teeth are lost, bone will resorb and mechanical stress is no longer applied to the bone. Dental implants preserve surrounding bone through function.
    • Osseointegration refers to direct structural and functional union between the implant and healthy living bone, and indicates successful placement of the implant. No mobility of the implant is evident at the implant-soft-tissue interface.
    • There are three types of implants: subperiosteal, transosseous, and peri-implant. Transosseous implants are the most invasive and pass through the full thickness of the alveolar bone. Ceramic-coated materials and titanium alloy are often used for transosseous implants. Peri-implant implants are the most common type and are inserted into the bone just below the gum line. Stainless steel and titanium alloy are the most common materials used for peri-implant implants.
    • The endosseous implant is the most common type of implant and is placed within the bone to replace a single tooth or provide support for the replacement of complete or partial loss of teeth.
    • Endosseous implants come in a variety of forms, including blade or plate form.
    • Tobacco use may adversely affect wound healing and jeopardize the success of dental implants and bone health.
    • Alcohol abuse use may cause changes in alveolar bone healing impacting osseointegration and may result in implant failure.
    • Poorly controlled diabetes mellitus impacts osseointegration and increases risk for peri-implantitis and implant failure.
    • Osteoporosis may have some impact on bone healing and osseointegration, but of greater concern is the bisphosphonate medication that patients may be taking to treat osteoporosis.
    • Patients using anticoagulant medications require consultation with the primary care provider to evaluate suitability for a dental implant.
    • Immunosuppressant therapy may also negatively impact healing.
    • There are a few conditions in which dental implants are not recommended which include: liver disease, renal disease, endocrine disorders, connective tissue diseases, and autoimmune diseases.
    • Periodontal disease and restorative status of all teeth need to be evaluated prior to placement of an implant.
    • Radiographic appearance of the implant and surrounding alveolar bone, occlusal evaluation including assessment of mobility, and peri-implant probing are all periodic evaluations that should be performed on the implant.
    • The seventh European Workshop of Periodontology consensus report and 2017 World Workshop on the Classification of Periodontal and Peri-Implant Diseases and Conditions indicates that probiotics may be beneficial for the treatment of peri-implantitis.
    • Peri-implant health is important and requires examination with a light touch to detect signs of infection or pathology.
    • The depth of the peri-implant pocket should be less than 5 mm to indicate good health.
    • Bleeding and suppuration are signs of pathology and must be monitored.
