Return-to-Work from a Medical Aid Injury/Illness

  • Employee is responsible for returning the completed Return-to-Work Form to their immediate Supervisor before the next scheduled shift.
  • The TFS Medical Office reviews the Return-to-Work Form to determine if any accommodation is necessary.
  • If no accommodation is required, the employee returns to their regular duties.
  • If accommodation is required, the TFS Medical Office assigns alternate duties.

Supervisor Responsibilities for a Lost Time Injury/Accident

  • Provide immediate first aid treatment as necessary.
  • Determine if the employee needs an ambulance or safe transport to a medical facility.
  • Have the employee complete a WSIB Form 6, if capable.
  • Provide the employee with Return-to-Work information, including:
    • Employee accident reporting guidelines
    • Return-to-Work Form
  • Conduct a detailed investigation of the accident/injury, including:
