Return-to-Work from a Medical Aid Injury/Illness
- Employee is responsible for returning the completed Return-to-Work Form to their immediate Supervisor before the next scheduled shift.
- The TFS Medical Office reviews the Return-to-Work Form to determine if any accommodation is necessary.
- If no accommodation is required, the employee returns to their regular duties.
- If accommodation is required, the TFS Medical Office assigns alternate duties.
Supervisor Responsibilities for a Lost Time Injury/Accident
- Provide immediate first aid treatment as necessary.
- Determine if the employee needs an ambulance or safe transport to a medical facility.
- Have the employee complete a WSIB Form 6, if capable.
- Provide the employee with Return-to-Work information, including:
- Employee accident reporting guidelines
- Return-to-Work Form
- Conduct a detailed investigation of the accident/injury, including:
- Visiting the accident/injury location
- Questioning witnesses and completing a Witness Report Form
- Completing a Supervisor’s Report of Injury/Incident and Quatro Safety entry
- Forwarding a copy to the District Chief
Return-to-Work From a Lost Time Injury/Accident
- The employee is responsible for returning the completed Return-to-Work Form to the TFS Medical Office.
- The TFS Medical Office reviews the Return-to-Work Form to determine if any accommodation is necessary.
- The Supervisor reviews the Return-to-Work Form to determine the possible Return-to-Work date and necessary accommodation.
- If no Return-to-Work is indicated, the PEHR Disability Management Consultant is notified.
- The Supervisor maintains regular contact with the injured employee for the first three months and then at suitable intervals.
- The PEHR Disability Management Consultant maintains regular contact with the Supervisor, the employee, treating physician/agency, and the Workplace Safety and Insurance Board until there is a successful resolution.
