Study Notes
Protozoology Overview
- Study of protozoa, unicellular organisms with diverse shapes, sizes, and locomotion methods.
- Protozoa can be pathogenic, commensal, or free-living.
Types of Protozoa
- Intestinal Amoeba:
- Pathogenic forms cause disease.
- Commensal forms coexist without harm.
- Free-living Amoeba can also be pathogenic.
- Flagellates and Ciliates represent motile protozoa.
- Includes Malarial Parasites and Coccidia.
Protozoa Structure
- Protoplasm consists of cytoplasm (ectoplasm and endoplasm) and nucleus.
- Ectoplasm:
- Contains locomotor apparatus (pseudopodia, flagella, cilia).
- Cytostome for food uptake and cytopyge for waste excretion.
- Endoplasm:
- Houses vital structures like ribosomes, mitochondria, nucleus, lysosomes, and vacuoles for osmotic regulation.
Life Cycle Stages
- Two primary stages:
- Trophozoite: active, motile, feeding state.
- Cyst: dormant, infective stage (except Trichomonas).
Reproduction
- Asexual reproduction through binary fission is common.
- Sexual reproduction, known as syngamy, occurs in some protozoan species.
Entamoeba histolytica
- Transmission via ingestion (fecal-oral) or sexually among specific groups (e.g., male homosexuals).
- Infective stage is a mature cyst with four nuclei.
Trophozoite Characteristics
- Size: 12-30 micrometers, with notable pseudopodia and a single, often unclear nucleus.
Cyst Characteristics
- Size: 12-20 micrometers; contains two nuclei; identified by reddish-purple chromatoid bodies and characteristic nuclear features.
Pathogenesis of Amoebiasis
- Non-invasive: Asymptomatic cyst passers reside in intestinal mucosa.
-
Invasive:
- Causes necrosis, leading to ulcers, severe dysentery, and possible extraintestinal amebiasis.
- Symptoms can involve dysentery, abscesses, and systemic infections.
Geographic Distribution
- Higher incidence in developing countries; risk groups include male homosexuals and travelers.
- Complications may include abscess formation and intestinal perforation.
Extraintestinal Amebiasis
- Primarily affects the liver (via portal vein) and occasionally other organs.
- Commonly observed are amebic liver abscesses characterized by chocolate-colored pus and necrotic material.
Diagnosis
- Based on symptoms, patient history, liver size, imaging techniques, and aspiration of abscess for analysis.
Treatment
- Asymptomatic cases: Iodoquinol or Paromomycin.
- Symptomatic cases: Metronidazole or Tinidazole; drainage of liver abscess may be necessary.
Prevention and Control
- Key measures include avoiding fecal-oral transmission, particularly during travel.
Non-Pathogenic Amoebas
- Include species like Entamoeba dispar, E. hartmanii, E. coli, and others with distinct morphological characteristics.
Commensal Amoeba
- Entamoeba dispar: Morphologically similar to E. histolytica but non-pathogenic.
- Entamoeba coli: Features include broad pseudopodia and a larger cyst with up to 16 nuclei.
- Entamoeba hartmanni and E. polecki: Smaller cysts with specific nuclear structures, with E. polecki primarily found in pigs and monkeys.
