Periodontal Health

  • A healthy periodontium is characterized by the absence or presence of up to two sites of clinically detectable inflammation, minimal or no bleeding on probing, and normal probing depths (≤3 mm).
  • The periodontium can be intact (no attachment or bone loss) or reduced (previous attachment or bone loss).

Gingivitis

  • Gingivitis is characterized by inflammation, erythema, edema, bleeding on probing, and normal probing depths (≤3 mm).
  • The periodontium is stable with no current progressive attachment or bone loss.

Periodontitis

  • Periodontitis is characterized by loss of periodontal support due to microbial-associated, host-mediated inflammation.
  • It is marked by deeper probing depths (≥4 mm) and presence of interproximal attachment loss from the current disease state.
  • This does not include attachment loss from a history of disease in currently stable patients.

Periodontal Staging

Stage I

  • Characterized by 1-2 mm greatest interdental clinical attachment loss.
  • Less than 10% bone loss.

Stage II

  • Characterized by 3-4 mm greatest interdental clinical attachment loss.
  • 10-20% bone loss.

Stage III

  • Characterized by 4-5 mm greatest interdental clinical attachment loss.
  • 20-30% bone loss.

Stage IV

  • Characterized by ≥5 mm greatest interdental clinical attachment loss.
  • More than 30% bone loss.
