Probing depth

Distance from the gingival margin to the bottom of the pocket

Six Point Pocket Chart

A chart with six specific points around each tooth used to record periodontal data.

Free Gingival Margin

The edge of the gingiva nearest the incisal or occlusal surface, not attached to the tooth.

Study Notes

Intended Learning Outcomes

  • Define the term probing depth
  • Revise components of periodontal probing
  • Revise the limitations of periodontal probing
  • Explain the location and method of undertaking a six point pocket chart
  • Describe the locations of the free gingival margin
  • Explain how to measure the free gingival margin
  • Explain Clinical Attachment Level and its relevance in the periodontal assessment
  • Explain how to calculate clinical attachment levels

Probing Depth

  • Probing depth refers to the distance from the gingival margin to the bottom of the pocket
  • It is measured to the nearest millimeter using a graduated periodontal probe with a standardised tip diameter of 0.4mm to 0.5mm

Six Point Pocket Chart

  • Distobuccal line angle to midline of distal surface
  • Buccal surface
  • Mesiobuccal line angle to mesial surface
  • Distolingual line angle to distal surface
  • Lingual surface
  • Mesiolingual angle to midline of mesial surface

Free Gingival Margin

  • Normal state: gingival margin is slightly coronal to CEJ and is recorded as zero (0)
  • Negative number (-'ve): gingival margin significantly covers CEJ
  • Positive number (+'ve): gingival margin located apical to CEJ

Clinical Attachment Levels (CAL)

  • CAL is a measure of the true periodontal support around a tooth
  • It provides an estimate of true periodontal stability and loss of support
  • CAL is measured from a fixed point on the tooth that does not change such as the CEJ
  • CAL is critical in distinguishing between gingivitis and periodontitis
  • Inflammation with no clinical attachment loss is characteristic of gingivitis
  • Inflammation with clinical attachment loss is characteristic of periodontitis

Calculating CAL

  • Probing depth and the level of the gingival margin is needed to calculate the CAL
  • When the gingival margin is at a natural location: CAL = Probing depth (6mm) - Gingival margin level (0mm) = 6mm
  • When recession is present with the gingival margin apical to CEJ(+'ve): CAL = Probing depth (4mm) + Gingival margin level ((+)2mm) = 6mm
  • When the gingival covers the CEJ (-'ve): CAL = Probing depth (9mm) - Gingival margin level ((-)3mm) = 6mm
  • Assessing true periodontal support around the tooth is needed to decipher the information obtained.
