Study Notes

Intestinal Flukes

  • Fasciolopsis buski: Large intestinal fluke that causes Fasciolopsiasis, transmitted through ingestion of infected water plants with metacercariae by humans or pigs.
  • Symptoms: Abdominal discomfort, irritation, inflammation, bleeding, jaundice, diarrhea, gastric discomfort, edema, and malabsorption syndrome.
  • Treatment: Praziquantel.

Liver Flukes

  • Fasciola hepatica: Sheep liver fluke that causes Fascioliasis, transmitted through ingestion of metacercariae on vegetation by definitive hosts.
  • Symptoms: Headache, fever, chills, pain in the liver area, eosinophilia, jaundice, liver tenderness, anemia, diarrhea, and biliary obstruction.
  • Treatment: Praziquantel or bithionol.

Chinese Liver Fluke

  • Clonorchis sinensis: Causes Clonorchiasis, transmitted through ingestion of pickled freshwater fish and shrimp.
  • Symptoms: Fever, abdominal pain, eosinophilia, diarrhea, anorexia, epigastric discomfort, occasional jaundice, and enlarged and tender liver.
  • Treatment: Praziquantel or albendazole.

Heterophid Flukes

  • Heterophyes heterophyes: Heterophid fluke that causes Heterophyiasis, transmitted through ingestion of undercooked fish containing metacercariae.
  • Symptoms: Abdominal pain, eosinophilia, diarrhea, and gastrointestinal discomfort.
  • Treatment: Praziquantel.

Oriental Lung Fluke

  • Paragonimus westermani: Causes Paragonimiasis and pulmonary distomiasis, transmitted through ingestion of freshwater products.
  • Symptoms: Cough, fever, chest pain, blood-tinged sputum, chronic bronchitis, eosinophilia, and fibrous tissue production.

Blood Flukes

  • Schistosoma spp.: Causes Schistosomiasis, bilharziasis, swamp fever, and Katayama fever, transmitted through cercariae penetrating the skin.
  • Symptoms: Abdominal pain, fever, chills, cough, diarrhea, eosinophilia, and painful urination and hematuria.
  • Treatment: Praziquantel, oxamniquine, or surgery.

Egg Characteristics

  • Fasciolopsis buski: Somewhat oblong eggs, 128-140 um by 78-85 um, with undeveloped miracidium and present operculum.
  • Fasciola hepatica: Somewhat oblong eggs, 128-150 um by 60-90 um, with undeveloped miracidium and present operculum.
  • Clonorchis sinensis: Eggs are 78-120 um long and 45-60 um wide, with undeveloped miracidium.
  • Heterophyes heterophyes: Eggs are 30 um by 15 um, with developed miracidium and present operculum.
  • Metagonimus yokogawai: Eggs are 30 um by 15 um, with developed miracidium and present operculum.
  • Paragonimus westermani: Eggs are 30 um by 15 um, with developed miracidium and present operculum.
  • Schistosoma spp.: Eggs are 112-182 um by 40-75 um, with developed miracidium and lateral spine.
