Paliperidone (Invega) Overview
- Classified as a benzisoxazole antipsychotic, under Category C for pregnancy.
- Administered orally (PO).
Indication
- Primarily indicated for the treatment of schizophrenia.
Side Effects
- Serious risks include neuroleptic malignant syndrome (NMS), tardive dyskinesia (TD), and extrapyramidal symptoms (EPS).
- Common side effects: drowsiness, headache, anxiety, confusion, dizziness, fatigue, and weakness.
- Cardiovascular effects: palpitations, increased/decreased heart rate, and hypotension.
- Gastrointestinal disturbances: abdominal pain, dry mouth, dyspepsia, nausea, and swollen tongue.
- Others include blurry vision, dyspnea, fever, dystonia, akathisia, dyskinesia, and hyperglycemia.
Nursing Responsibilities
- In elderly patients with dementia, use is contraindicated.
- Not recommended for pediatric populations.
- Essential to monitor blood pressure, pulse, ECGs, and cardiovascular symptoms.
- Regularly assess for EPS, TD, NMS, and monitor weight, BMI, fasting blood sugar (FBS), and lipid levels.
- Be vigilant for signs of priapism.
Drug Action
- Functions by antagonizing dopamine and serotonin receptors.
Contraindications
- Avoid use in individuals with hypersensitivity or concurrent medications causing QT prolongation.
- Contraindicated in patients with a history of QT prolongation, cardiac arrhythmias, lower heart rate, hypokalemia, hypomagnesemia, or gastrointestinal narrowing.
Caution
- Use with caution in patients with Parkinson's disease or dementia with Lewy bodies.
- Individuals with a history of suicide attempts or those at risk for aspiration pneumonia require careful monitoring.
- Caution for patients with a history of seizures, conditions increasing temperature, intestinal obstruction, and Reye's syndrome.
- Evaluate risks in patients with brain tumors, diabetes, hepatic impairment, or renal impairment.
Interactions
- CNS depression may be increased with alcohol, antihistamines, sedatives, and opioid analgesics.
- Can antagonize the effects of levodopa or other dopamine agonists.
- Risk of lower blood pressure is heightened when used with antihypertensive agents or nitrates.
