Public Sector Employees
- Company Officers must contact the Illinois State Plan OSHA hotline or the Federal OSHA hotline for:
- Work-related fatalities within 8 hours of incident
- Work-related inpatient hospitalizations, amputations, and losses of an eye within 24 hours of incident
Private Sector Employees
- Company Officers must contact the Federal OSHA hotline or the Chicago North OSHA Area Office for:
- Work-related fatalities within 8 hours of incident
- Work-related inpatient hospitalizations, amputations, and losses of an eye within 24 hours of incident
Reporting Procedures
- The reporter of the incident must provide:
- Establishment name
- Location of the incident
- Time of the incident

- Brief description of the incident
- If possible, secure the scene until Illinois OSHA arrives, or take pictures and/or diagram the incident

- Private Sector Employee: works for a non-government employer, including individual business owners, corporations, or limited partnerships
