- Quality (Q): Encourage the patient to describe their pain using terms such as crushing, aching, dull, stabbing, or burning; utilize open-ended questions for clarity.
- Radiation (R): Assess the location of the pain and inquire if it radiates to other areas.
- Severity (S): Request the patient to rate their pain on a scale of 1 to 10, with 10 being the most severe.
