Omeprazole (PriLOSEC) Overview
- Generic name: omeprazole
- Trade name: PriLOSEC
- Classification: antiulcer agent (therapeutic); proton-pump inhibitor (pharmacologic)
Indications
- Used for Gastroesophageal Reflux Disease (GERD) and for maintaining healing in erosive esophagitis
- Addresses duodenal ulcers and provides short-term treatment for active benign gastric ulcers
- Available OTC for heartburn occurring more than twice weekly
Mechanism of Action
- Binds to an enzyme on gastric parietal cells at acidic pH
- Prevents transport of hydrogen ions into the gastric lumen, reducing acid accumulation
Therapeutic Effects
- Decreased acid levels in the gastric lumen
- Reduction in gastroesophageal reflux
- Promotes healing of duodenal ulcers
Laboratory Tests
- Regular monitoring of CBC with differential, liver enzymes (AST, ALT, alkaline phosphatase, bilirubin)
- Monitor INR and PT time in patients on warfarin
- Watch for hypomagnesemia
- Serum gastrin concentration may rise in initial weeks of treatment
Adverse Reactions
- Gastrointestinal risks include life-threatening C. Difficile Associated Diarrhea (CDAD)
- Common side effect: abdominal pain
Contraindications
- Avoid in patients with hypersensitivity to omeprazole
- Discontinue use in breastfeeding women
- Caution advised in patients with liver disease
- Long-term use (over one year) may heighten risk of hip, wrist, and spine fractures
- Extended use (over three years) increases risk of vitamin B12 deficiency
Nursing Implications
- Assess for signs of gastrointestinal bleeding (occult blood, emesis)
- Monitor bowel function closely; report diarrhea, cramping, fever, or bloody stools as they may indicate CDAD
Client Teaching
- Inform patients that drug may cause drowsiness or dizziness
- Advise to notify healthcare provider of all medications, vitamins, or supplements, especially St. John's Wort
- Recommend avoidance of alcohol, aspirin, NSAIDs, and irritating foods
- Instruct patients to report any black, tarry stool, diarrhea, abdominal pain, or persistent headaches, especially if stools contain blood, pus, or mucus
- Advise against self-treating diarrhea due to potential severity
