    Purpose and Scope

    • The purpose of this document is to provide a coordinated, communicated, and controlled safe system of work in hazardous/non-hazardous areas under KOC jurisdiction.
    • It aims to achieve a safe working environment by identifying and mitigating hazards that could affect personnel, the public, facilities, or the environment.

    Definitions

    • e-WP (e-Work Permit): a web-based solution to manage the authorization process of non-routine activities electronically.
    • Permit Applicant: a person representing the controlling team who raises a Work Permit and has the technical knowledge, skill, and competence to consider and guard against potential hazards associated with the work activity.
    • Permit Issuer: a person representing the asset operating team who issues a Work Permit and has a complete overview of all operational activities and detailed knowledge of the Permit to Work System.
    • Worksite Supervisor: a person authorized to supervise the work at the worksite, who has detailed knowledge of the Permit to Work System.

    Key Responsibilities

    Permit Issuers

    • Permit Issuers are responsible to ensure:

    • Worksite is safely prepared, examined, and all specified precautions have been taken before work begins
    • Compulsorily visit worksite before permit authorization
    • Work permits that may interact or affect one another are cross-referenced clearly
    • Worksite is examined to ensure it is in safe and acceptable condition
    • Shift change handover procedure is properly followed and permit renewal/endorsement and transfer of responsibilities have been completed
    • Prior to closing or cancelling a work permit, any precautions and isolations have been withdrawn and the system returned to normal operation
    • When work is completed and the work permit is signed off, Permit Issuer closes the work permit and returns the equipment to operation
    • Permit Issuers belonging to KOC Asset Owner will also identify critical works
    • Permit Issuers will ensure continuous KOC supervision of critical works by respective Controlling Team including follow-up by Permit Issuer on regular intervals
    • Review critical work’s task-based risk assessment JSA submitted by Permit Applicant

    Nominated Permit Issuers

    • The following authorities or subordinates may be nominated as Permit Issuers in writing by their Team Leader:
    • Shift Supervisor, Senior Engineer, Controller, Operator Meter Test (for Export Operations)
    • Port Captain, Harbour Master, Pilot, Foreman (for Port Operations & Fleet Management)
    • Snr. Marine Engineer, Marine Maintenance Engineer, Engineer Port Services, Supervisor, Foreman, Assistant Foreman, Snr. Technician (for Marine Maintenance & MOSM)
    • Oil Spill Officer, Engineer, Skipper, Section Head, Controller (Day/Night), Operator - II (for Production/ Gas/EPF Operations)
    • As mentioned in Appendix B (for Drilling Operations)

    Worksite Supervisor

    • The Worksite Supervisor is responsible to the Permit Applicant for all aspects of safety related to work and the work party.
    • The Worksite Supervisor shall ensure:
    • He has detailed working knowledge of procedures related to his work activity
    • He does not start any job requiring a work permit until it is authorized and issued
    • He understands the limitations and restriction of the work permit in order that work may proceed safely
    • All members of the work party adhere to safe working practices and are fully conversant with the limitations, restrictions, and hazards involved
    • He conducts toolbox talk and pre-job briefing to ensure all precautions specified in work permit are taken
    • He does not leave the worksite without transferring responsibility of work supervision during his absence
    • He initiates permit renewal ensuring the work permit is renewed/endorsed as necessary and responsibilities are passed to the oncoming worksite supervisor at the shift change handover
    • The job described on the work permit is completed and the worksite as well as equipment is left in a safe condition if not completed
    • On completion of the job, the work permit is signed off and passed to the Permit Applicant & Permit Issuer for the equipment/worksite to be returned to the asset owner
    • The original work permit along with all referred documents and relevant certificates is prominently displayed at the worksite at all times during the work
    • The Worksite Supervisor will print a copy of the issued e-WP and keep it at the site

    Nomination / Authorization of Personnel for Permit to Work System

    • All personnel associated with Permit to Work System shall fulfill the following competency requirements before being authorized for their stipulated role:
    • In addition to his pre-requisite educational qualification and experience for the job, he must have detailed knowledge of the ‘Permit to Work System’ and “Job Safety Analysis”
    • Permit Issuers belonging to Asset Owner/Operations Team shall undergo PTW training followed by qualifying test, which will be conducted by OTPD Team in accordance with training material and test questionnaires provided by HSE Group
    • Permit Issuers other than Asset Owner/Operation Teams, Permit Applicant and Worksite Supervisor must have successfully completed training on ‘Permit to Work System’ and have passed the qualifying test (written as well as oral) relevant to his role
    • The concerned controlling team, which has ultimate responsibility for safe execution of work in their area of jurisdiction, shall arrange such training on PTW and qualifying tests for respective workers as per their stipulated role

    Directorate / Asset HSE Teams

    • The Directorate/Asset HSE Teams are responsible for:
    • Cross-verifying that the control measures mentioned in the permit are implemented for all critical activities in their respective area
    • Carrying out periodical HSE Inspections to ensure implementation of PTW Procedure requirements
    • Conducting daily random checks on active work permits and communicating deficiencies to respective Permit Issuer immediately
    • Authorised to stop work in case of work permit violations at worksite
    • Nominating their employees to participate as auditor/lead auditors for PTW audits being managed by HSE Group as per approved KOC HSE Annual Audit Register### Permit to Work Procedure

    Timing of Meeting

    • The meeting should be held 24 hours before the planned work, involving the Permit Applicant, Permit Issuer, Worksite Supervisor, and Contractor Safety Officer
    • The respective asset Safety Engineer/Officer may be invited depending on the work's criticality

    Types of Work Permits

    • There are 5 types of Work Permits:
      • Cold Work Permit
      • Hot Work Permit
      • Radiography Permit
      • Vehicle / Mobile Plant Entry Permit
      • Confined Space Entry Permit
    • Note: Confined Space Entry Permit and Vehicle / Mobile Plant Entry Permit are only valid for entry purposes; additional permits are required for work inside the confined space or facility

    Work Permit Validity

    • Hot / Cold / Radiography Permits: valid for a maximum of 7 consecutive days from the date of issue, subject to renewal by the Permit Issuer
    • Confined Space Entry Permit: valid for one continuous work shift or part thereof
    • Vehicle / Mobile Plant Entry Permit: valid for one continuous work shift for Vehicle entry, and a maximum of 7 consecutive working days for Mobile Plant entry

    Work Permit Application

    • All identified work must be authorized and planned prior to work permit application
    • Separate work permits must be applied for each activity, equipment, or location
    • The Permit Applicant must raise the appropriate work permit with a full description, including:
      • The Worksite
      • The Equipment to be worked on
      • The exact nature and scope of work (supported with sketches or drawings if available)
      • Special tools or equipment that may impact safety at the worksite

    Work Permit Authorization

    • The Permit Issuer reviews the work permit and supporting documentation, and completes the appropriate sections
    • The Permit Issuer arranges for equipment preparation, including depressurizing, draining, flushing, and purging, and ensures that safety precautions are in place

    Work Permit Issuance
