Dosage Forms

- Powder for injection: Freeze-dried, 20 mg and 40 mg in single-dose vials
Indications
- Short-term treatment of erosive esophagitis
- Management of symptomatic gastroesophageal reflux disease (GERD)
- Adjunctive use in triple therapy for H. pylori infection
- Reduction of gastric ulcers
- Treatment of pathological hypersecretory conditions (Zollinger-Ellison Syndrome)
Dosing Information
- Normal oral starting dose: 20-40 mg daily for 4-8 weeks
- GERD dosing: 20 mg daily for 4-8 weeks
- Erosive esophagitis and gastric ulcer maintenance: 20 mg daily for 4-8 weeks
- H. pylori treatment: 40 mg daily with 500 mg clarithromycin and 1000 mg amoxicillin twice daily for 10 days
- Gastric ulcer occurrence reduction: 20-40 mg once daily for up to 6 months
- Short-term GERD treatment via IV: 20-40 mg administered over at least 3 minutes or as a continuous infusion over 10-30 minutes
- Pathological hypersecretory conditions: 40 mg twice a day
Mechanism of Action
- Proton pump inhibitor that suppresses the H+/K+ ATPase enzyme system in gastric parietal cells, blocking the final step of acid production.
Pharmacokinetics
- Peak plasma level: Achieved in approximately 1.5 hours
- Terminal elimination half-life: Also 1.5 hours
- Metabolism: Primarily via CYP2C19 isoenzyme, with some metabolism by CYP3A4 isoenzyme
Drug Interactions
- Drugs requiring gastric pH may have impaired absorption (e.g., ketoconazole, digoxin)
- Increases plasma levels of diazepam
- Decreases plasma levels of atazanavir
- Inhibits metabolism of cilostazole
Contraindications and Precautions
- Long-term use may increase the risk of gastric carcinoid tumors
- Rare severe dermatologic reactions (e.g., toxic epidermal necrolysis)
- Classified as Pregnancy category B
Adverse Effects
- Common side effects include headache, diarrhea, constipation, nausea, gas, abdominal pain, and dry mouth
Patient Consultation Points
- Take at least 1 hour prior to eating
- Swallow capsules whole or sprinkle enteric-coated granules on applesauce and swallow immediately
- Mix granules with 15 mL of water, stir, let sit for 3 minutes, take within 30 minutes of mixing
- Antacids can be taken concurrently
- Store in a cool, dry place
- If a dose is missed, take as soon as possible or skip if near the next dose
- Do NOT double doses
- Contact a physician for persistent side effects
