- Registration is free and subject to department approval.
- Registration is valid for two years.

- The number of prescriptions requested is subject to department approval and is issued free of charge.
- Forms are sent to the address listed on the practitioner's Federal Drug Enforcement Administration (DEA) registration.

- The commissioner may cancel forms at any time by giving notice to the practitioner.
Revocation and Return of Prescription Forms
- The commissioner may revoke, cancel, or withhold prescription forms for willful failure to comply with the Public Health Law or this Title.
- Practitioners who are deceased or no longer prescribe controlled substances must return official prescription forms to the Bureau of Narcotic Enforcement, NYS Department of Health.
