Chief Complaint and History of the Present Illness
- Chief Complaint (CC) is a concise statement including the patient's age, sex, and presenting problem (e.g., "42-year-old female with hip pain after a fall").
- History of Present Illness (HPI) describes the complete history of the current medical problem;
- It includes potential risk factors, chronological development, previous care, and related conditions.
- HPI should include negative information to rule out other conditions.
Past Medical History and Review of Systems
- Past Medical History (PMH) includes non-related medical conditions. For example, "a 6-year history of mild hypertension."
- Review of Systems (ROS) is a brief review of all medical systems, including HEENT, Cardiopulmonary, GI, GU, OB/GYN, dermatologic, neurologic, musculoskeletal, psychiatric, hematological, oncologic, rheumatological, and infectious diseases.
Family History, Social History, and Medications & Allergies
- Family History (FHx) includes a brief synopsis of any illness in immediate family members.
- Social and Environmental History (SocHx/EnvHx) includes occupation, family status, travel history, sexual history, social habits, and lifestyle choices.
- Medications and Allergies list current medications (including vitamins, supplements, and over-the-counter drugs) and known allergies.
Physical Exam
- The physical exam proceeds from head to toe and includes general appearance, vital signs (temperature, pulse, blood pressure, respiratory rate), HEENT (head, eyes, ears, nose, and throat), neck, back and spine, lymph nodes, breasts, lungs, heart, abdomen, extremities, pulses, neurologic, rectal, pelvic and genitalia, and dermatologic examinations.
Laboratory Data and Assessment & Plan
- Laboratory Data includes all diagnostic tests (blood work, CSF/urine tests, electrocardiogram, radiological tests).
- Assessment and Plan begins with a summary of the likely diagnosis (and a differential diagnosis if uncertain).
- The Plan section details problems and proposed interventions/diagnostic procedures.
