Head and Neck Assessment
- Focuses on the cranium, face, thyroid gland, lymph nodes, eyes, ears, nose, and mouth
Skull Shape and Size
- Varies by age, gender, and race
- Abnormalities can be caused by congenital disorders, trauma, brain tumors, scalp swelling, hematoma, skin cancer, and hydrocephalus
- Microcephaly: abnormally small head
- Acromegaly: skull and facial bones are larger and thicker
Assessing the Head
- Normal findings: round, normal size or head circumference, smooth skull contour, no lesions
- Abnormalities: asymmetry, deformities, lacerations, signs of trauma
Assessing the Face
- Normal findings: symmetric, normal range of motion, no swelling or nodules
- Abnormalities: asymmetry, drooping, weakness, or paralysis on one side of the face
- Temporal arteritis: inflammation of the temporal artery, may lead to blindness
- TMJ syndrome: limited range of motion, swelling, tenderness, or crepitation
Assessing the Neck
- Normal findings: symmetric, no swelling, no lymph node enlargement
- Abnormalities: swelling, enlarged masses or nodules, inflammation, or bulging masses
- Jugular vein distension (JVD): indicates right-sided heart failure or cardiac problems
General Survey of the Client
- Focuses on levels of consciousness
- Normal response: alert, oriented, and responsive
- Abnormalities: disoriented, confused, lethargic, obtunded, stuporous, or comatose
