- Taking notes is recommended, especially if you're unfamiliar with the medications.
Medication Classes and Examples
- Spironolactone is a potassium-sparing diuretic; avoid extra potassium intake.
- Direct vasodilators include hydralazine.
- ACE inhibitors end in "pril" (e.g., lisinopril).
- Angiotensin receptor blockers (ARBs) end in "sartan" (e.g., losartan).
- Beta blockers end in "olol" (e.g., metoprolol).
- Furosemide is a potassium-wasting diuretic.
- Thiazide diuretics (e.g., hydrochlorothiazide/HCTZ) are also potassium-wasting.
Insulin and Hypoglycemia
- For regular insulin, monitor for hypoglycemia at its peak time.
- Regular insulin typically peaks at two hours.
- If regular insulin is administered at 0800, watch for hypoglycemia at 10:00 am
- Patients should eat 30 minutes after recieving regular insulin
Questioning vs. Clarifying Orders
- Questioning an order means identifying orders that could harm the patient.
- Clarifying an order involves identifying orders that are incorrectly written.
- Incorrectly written orders may be missing the route, the frequency, or have an unapproved abbreviation
- "qid" should be written as "four times a day" or "four times daily."
- Avoid trailing zeros after a decimal (e.g., 12 mg, not 12.0 mg).
- Leading zeros are necessary before a decimal (e.g., 0.1 mg).
Insulin Types and Administration
- Glargine has no peak and is given once or twice a day.
- Giving glargine AC (before meals) and HS (at bedtime) is incorrect.
- NPH insulin is intermediate-acting and can be given twice a day.
- NPH at bedtime should be given with a snack.
- Glargine can be given at bedtime without a snack
Metformin and Dye Procedures
- Metformin is contraindicated with any procedure involving dye (e.g., coronary angiogram).
- Any procedure with gram in the word indicates a dye
- General knowledge for nurses is that Metformin is contraindicated with dye procedures
Oxytocin and Fetal Heart Monitoring
- Late decelerations in fetal heart monitoring are a problem.
- The first response is to turn off the oxytocin.
- Then, turn the patient to her left side, apply O2, and notify the obstetrician.
- Early decelerations are fine, high baseline variability is good, and low variability/fetal heart rate is bad.
- Low fetal heart rate is under 110 bpm.
- Low heart rate variability, low fetal heart rate and late decelerations are bad
