Study Notes
General Protocol Guidelines
- Procedures marked with a black diamond require Medical Control Physician (MCP) authorization before execution.
- Items in braces ({ }) denote options available to the department and its Medical Director, allowing for flexibility in directives.
Patient Categories
- Pink color codes identify pediatric patients.
- Sections labeled with an "A" pertain to adult patients.
- Sections marked with a bold "G" indicate geriatric patients.
Communication with Hospitals
- Call the hospital to prepare for a patient in serious situations (e.g., trauma, stroke, cardiac arrest).
- Provide MIVT (Mechanism of Injury, Injuries, Vital Signs, Treatment) details for trauma patients when notifying ahead.
- Include components of the Glasgow Coma Scale (GCS) and findings from patient assessments during communication.
Trauma and Stroke Alerts
- Alerts for Trauma or Stroke should be communicated as "We recommend a __________ alert."
Non-Initiation of Care Circumstances
- Do not initiate care in cases of severe cranial injuries, massive truncal wounds, DNR orders, rigor mortis, or prolonged arrest scenarios.
- Triage demands and specific trauma circumstances (e.g., blunt trauma) can also justify non-initiation.
Exclusionary Conditions for Care
- Rapid transport is necessary for pregnant females with traumatic arrest after 24 weeks if a pulse is present.
- Conditions like witnessed arrest by EMS, hypothermia, or commotio cordis warrant initiating care despite other challenges.
DNR Definitions
- DNR-Comfort Care (DNR-CC): Treatment to alleviate pain without delaying death.
- DNR-Comfort Care Arrest (DNR-CCA): All protocol-based treatments until cardiac or respiratory arrest occurs.
SAMPLE Assessment Acronym
- Stands for: Symptoms, Allergies, Medications, Past medical history, Last oral intake, Events leading to injury.
Oral Glucose Indication
- Administer for conscious but disoriented patients with blood sugar below specific thresholds.
Spinal Motion Restriction
- Full spinal motion restriction is applicable in cases when patients are classified from V3 to V6 or V5.
ECG Lead Placement
- V1: 4th intercostal space, right of the sternum bump.
- V2: 4th intercostal space, left of the sternum.
- V3: Midway between V2 and V4.
- V4: 5th intercostal space, midclavicular line from V2.
- V5: Lateral to V4, along the arm-chest line.
- V6: Midaxillary line, level with V5.
12-Lead Preparation and Communication
- Prepare skin by shaving excess hair and ensuring dryness.
- Instruct the patient to remain still, minimizing movement to ensure accurate readings.
- Steps for transmitting a 12-Lead include skin preparation, lead placement, noting patient’s age/gender, sending to selected hospital, and informing them of MIVT and ETA upon arrival.
