Mental Health Assessment & Cognitive Function
- Focus must be aimed at memory recall, orientation and attention span when assessing cognitive function.
- Thought process, appearance, cognition and behavior are all part of the mental status assessment.
- Affect is the outward expression of emotion, such as facial expressions or tone
- Mood is the client's emotional state
Therapeutic Communication
- Open-ended questions are effective techniques, for example “Tell me more about what you are feeling."
- Avoid directive or judgmental language and use reflection, summarization and active listening instead.
- Feelings should not be minimized, and interruptions or false reassurances, for example “Don't worry, everything will be fine” should be avoided.
Legal & Ethical Considerations
- Refusing treatment is a right of even involuntarily committed patients, unless treatment is court-ordered.
- If a client threatens to harm others, confidentiality can be breached due to duty to warn.
- Unjustified restraint of a client is false imprisonment.
- Physically touching a client without their consent is battery.
- Threatening a client with harm is assault.
- Objective language should always be used in documentation, for example, "Client hit another patient and shouted 'I will hurt you.'" and not "Client was aggressive."
Mental Health Disorders & Treatment Approaches
- CBT or Cognitive Behavioral Therapy focuses on identifying and modifying negative thought patterns.
- Monitoring for side effects and medication administration are psychobiological interventions.
- A structured environment that promotes social interaction and healing is a therapeutic milieu.
Crisis Management & Safety
- When handling aggression, ensure safety first, use de-escalation techniques, and avoid confrontation.
- Seclusion & Restraints should be a last resort, continuous monitoring is required in addition to a provider's order.
- Emergency Admission occurs when there is imminent danger to others or to one-self.
Medication Safety & Pharmacology
- SSRIs (e.g., fluoxetine) may require 4-6 weeks to take full effect.
- Tyramine-rich foods like salami, red wine, and aged cheese should be avoided when taking MAOIs (e.g., phenelzine) in order to avoid hypertensive crisis.
- Orthostatic hypotension, weight gain and dry mouth are side effects of tricyclic antidepressants (e.g., amitriptyline).
- For Lithium, monitor for toxicity. Severe diarrhea, blurred vision, and muscle weakness are signs of toxicity.
- Haloperidol can cause extrapyramidal symptoms (EPS) that include tardive dyskinesia, shuffling gait, and muscle rigidity.
- Anxiolytics (e.g., alprazolam) have a risk of dependency that can be prevented by avoiding alcohol to prevent CNS depression, and avoiding sudden stops.
- Stimulants (e.g., methylphenidate for ADHD) can cause insomnia, increased BP, and weight loss.
- Sedative-hypnotics (e.g., zolpidem for insomnia) should be taken before bed, as daytime drowsiness is a possible side effect.
Maslow's Hierarchy in Nursing Care
- Food, rest, warmth, and water fulfill basic physiological needs.
- Ensuring a client does not harm themselves or others fulfills safety needs.
- After physiological and safety needs are met, self-esteem and self-actualization interventions can occur.
Dosage Calculation Review
- Amount to give = (Ordered dose / Available dose) × Volume for dose calculation.
- IV flow rate equates to mL/hr = Total volume in mL / Time in hours
- Drop factor calculation formula is gtt/min = (Volume in mL × Drop factor) / Time in minutes
- For example: 2.5 mL is the result of Ordered: 250 mg, Available: 500 mg/5 mL
- Also: 22 gtt/min is the result of Haloperidol 1g in 100 mL over 45 min, Drop factor: 10 gtt/mL
