  • Focus must be aimed at memory recall, orientation and attention span when assessing cognitive function.
  • Thought process, appearance, cognition and behavior are all part of the mental status assessment.
  • Affect is the outward expression of emotion, such as facial expressions or tone
  • Mood is the client's emotional state

Therapeutic Communication

  • Open-ended questions are effective techniques, for example “Tell me more about what you are feeling."
  • Avoid directive or judgmental language and use reflection, summarization and active listening instead.
  • Feelings should not be minimized, and interruptions or false reassurances, for example “Don't worry, everything will be fine” should be avoided.
  • Refusing treatment is a right of even involuntarily committed patients, unless treatment is court-ordered.
  • If a client threatens to harm others, confidentiality can be breached due to duty to warn.
  • Unjustified restraint of a client is false imprisonment.
  • Physically touching a client without their consent is battery.
  • Threatening a client with harm is assault.
  • Objective language should always be used in documentation, for example, "Client hit another patient and shouted 'I will hurt you.'" and not "Client was aggressive."
