Mandibular Limiting Structures
- The labial frenum is a fold of mucous membrane that limits the denture base and should be notched to allow movement.
- The labial vestibule limits the denture flange thickness and length.
- The buccal frenum is a fold of mucous membrane in the premolar area that requires a notch in the denture to accommodate its movement.
- The buccal vestibule is related to the buccinator muscle; lower denture flanges should extend in the buccal vestibule as it doesn't displace the denture.
- The masseter muscle influencing area, or masseteric notch, requires contouring of the distobuccal flange to allow muscle movement without denture displacement.
- The posterior end of the retromolar pad is the posterior limit of the lower denture, and postdamming can be performed here for retention.
- The palatoglossal arch forms the disto-lingual border of the lower denture, and overextension can cause sore throat.
- The lingual pouch is a space bordered posteriorly by the palatoglossus muscle, anteriorly by the mylohyoid muscle, medially by the tongue, and laterally by the medial aspect of the mandible.
- The mylohyoid muscle influencing area, or internal oblique ridge, is located on the medial surface of the mandible and requires consideration for denture design.
- The sublingual salivary gland area is a potential area for denture overextension, which can cause discomfort and displace the denture.
- The lingual frenum is a fold of mucous membrane that connects the tongue to the floor of the mouth. A notch is needed in the denture to accommodate its movement.
Mandibular Intra-oral Landmarks
- The residual alveolar ridge is the remaining portion of the alveolar process after tooth extraction. Severe resorption requires special impression techniques or surgery for proper denture stability.
- The external oblique ridge is a dense bony ridge that descends obliquely from the ramus of the mandible. The lower denture should cover but not extend beyond this ridge to avoid displacement.
- The buccal shelf of bone is bordered externally by the external oblique line and internally by the residual ridge. It is a primary stress bearing area in the mandibular arch.
- The mental foramen is the anterior exit of the mandibular canal and contains the mental nerve. The denture base must be relieved in this area to prevent nerve compression and pain.
- The retromolar pad is a pear-shaped pad of soft tissue located at the distal end of the residual ridge. It contains mucous glands, tendons, and muscle fibers, and should be covered by the denture for retention and cushioning.
- The torus mandibularis is a bony prominence often found on the inner surface of the mandible, typically in the premolar region. Relief of the denture base may be required to avoid impingement and discomfort. If it is large and impedes denture placement, surgical intervention is necessary.
- The mylohyoid ridge is a bony ridge extending on the medial surface of the mandible, from the third molar region to the lower border of the mandible. It's position varies with the degree of alveolar ridge resorption.
- The genial tubercles are found on the medial surface of the mandible and serve as attachment sites for the genioglossus and geniohyoid muscles. In cases of severe ridge resorption, they may cause discomfort if exposed to the denture base.
