Male Genital System

Scrotum

  • Consists of superficial fascia, which includes:
    • Superficial fatty layer replaced by Dartos muscle
    • Deep membranous layer (Colle's fascia) continuous with deep membranous layer of ant. abd. wall (Scarpa's fascia)
  • Lymphatic drainage → Superficial inguinal LN

Testis

  • Oval shape
  • Coverings: 3 coats from outside inwards:
    • Tunica vaginalis: has visceral and parietal layers, covers the whole testis except posterior border
    • Tunica albuginea: fibrous coat all around testis
    • Tunica vasculosa: innermost vascularized C.T.
  • Blood supply: Testicular artery from abdominal aorta
  • Venous drainage: Pampiniform plexus of veins → testicular v. → Rt. testicular vein → Inferior Vena Cava, Lt. testicular vein → Left renal vein
  • Lymph drainage: Pre-aortic & para-aortic L.N.

Epididymis

  • Forms a cap on the upper pole of testis, descends lateral to posterior border till its lower pole
  • Highly coiled duct, approximately 6 meters long when uncoiled
  • Parts:
    • Head: forms a cap on the upper pole of testis
    • Body: formed of a single coiled tube
    • Tail: lower end of epididymis, continues as the vas deferens
  • Function: reservoir for sperms

Spermatic Cord

  • Constituents: vas deferens, duct of seminal vesicle, and nerves
  • Coverings: spermatic fascia, cremasteric fascia, and external spermatic fascia

Vas Deferens

  • Begins at the epididymis, crosses the ureter, and descends medial to seminal vesicle
  • Terminal part is dilated to form the ampulla of the vas deferens, which joins the duct of seminal vesicle to form the ejaculatory duct

Prostate Gland

  • Conical shape, with a base above, an apex below, and 4 surfaces (anterior, posterior, and 2 inferolateral)
  • Position: lies immediately inferior to the neck of the bladder and anterior to the rectum
  • Lobes: divided into 5 lobes by the urethra and the 2 ejaculatory ducts:
    • Anterior lobe
    • Posterior lobe (usual site for cancer)
    • Right and left lateral lobes (usual site for senile enlargement)
    • Median lobe
  • Blood supply: branches from inferior vesical, middle rectal, and internal pudendal arteries
  • Veins: prostatic venous plexus drains into internal iliac veins
  • Lymph drainage: internal iliac nodes and sacral lymph nodes
  • Nerve supply: from inferior hypogastric plexus
  • Clinical note: a venous communication is present between prostatic venous plexus and internal vertebral venous plexus, explaining why cancer prostate usually sends metastases to the vertebral bodies.
